目前,对丙型肝炎病毒抗体(抗hcv)检测的假阳性率很高。然而,在反射补充检测中使用信号截止(S/Co)比率可以降低假阳性率。在此,本研究分析了一项抗hcv试验的2年数据,以了解S/Co比值的重要性,并通过第二次抗hcv试验进行验证,得出一种新的算法。

本研究回顾了32,573个来自三级医院的化验样本。使用Elecsys(罗氏诊断)Vitros (Ortho临床诊断)346例抗hcv阳性样本进行了再检测。分别对147例和11HCV阳性标本进行HCV RNA PCR和重组免疫印迹检测(RIBA)

研究结果显示,在32,573份样本中,446(1.37%)为阳性,32,127(98.6%)为阴性。与Architect-ElecsysArchitect-VitrosElecsys-Vitros比较,低S/Co样本(0.9-10.0)的一致性率分别为35.2%43.8%81.9%S/Co比值的相关系数为:Architect-Elecsys, 0.20;Architect-Vitros 0.42;Elecsys-Vitros, 0.46。在logistic回归中,预测阳性率95%S/Co值为3.13,预测阳性率50%S/Co值为8.85S/Co比值为1.70-3.34,表示5种抗原中有1种为阳性反应,S/Co比值为13.54-17.72,表示5种抗原中有3 - 5种为阳性反应。

研究结果表明,抗hcv筛查结果的补充检测对于区分抗hcv的真阳性和假阳性是非常必要的。本研究中提出了一种二次试剂复测辅助检测的算法,对临床实验室有一定的参考价值。
