非酒精性脂肪肝疾病(NAFLD)的发病率和流行率在全球范围内迅速上升。据统计,全球NAFLD的患病率约为25%,从非洲的13%到东南亚的42%不等。NAFLD包括一系列疾病,包括非酒精性脂肪肝(NAFL)或单纯性脂肪变性(该病程较温和),以及非酒精性脂肪性肝炎(NASH),后者可发展为肝硬化甚至肝细胞癌(HCC)。

有研究分析,预计2016年至2030年,中国、法国、德国、意大利、日本、西班牙、英国和美国的NAFLD患病率将增加高达56%。NAFLD由代谢综合征驱动,与肥胖、胰岛素抵抗和高脂血症有关。肥胖和胰岛素抵抗导致慢性炎症、脂质代谢改变并促进HCC发生、发展。

NAFLD包括几种组织学特征,从简单的脂肪变性到带有小叶炎症和气球状肝细胞的脂肪变性(脂肪性肝炎),这两种情况都可能伴有不同程度的纤维化。

目前,NAFLD公认的诊断参考标准是肝脏活检,因为其诊断特征是基于组织学的。然而,肝脏活检是有创的,有并发症的风险,同时受限于取样的变异性以及病理报告中观察者依赖性的高变异性。因此,如何更早、更好地识别NAFLD仍是摆在临床医生面前的一个难题。

为了评估通过振动控制的瞬时弹性成像(LSM-VCTE)、纤维化-4指数(FIB-4)和NAFLD(非酒精性脂肪肝)纤维化评分(NFS)的个体诊断性能,并得出可以减少肝脏活检的诊断策略,近期,国际LITMUS研究组开展了相关研究,结果发表在GUT杂志上。

研究人员对评估LSM-VCTE与肝脏组织学的研究进行了单个患者数据的荟萃分析。在可能的情况下计算FIB-4和NFS。计算敏感性、特异性和接收操作曲线下的面积(AUROC)。对生物标志物进行了单独和连续的组合评估。

共纳入37项主要研究(n=5735;45%为女性;中位年龄:54岁;中位BMI=30 kg/m2;33%患有2型糖尿病;30%患有晚期纤维化)。)总的来说,单个LSM-VCTE、FIB-4和NFS用于这怒单晚期纤维化的AUROCs为0.85、0.76和0.73。

依次结合FIB-4截止值(<1.3;≥2.67)和LSM-VCTE截止值(<8.0;≥10.0 kPa)来排除晚期纤维化,其敏感性和特异性(95% CI)可分别达到66%(63-68)和86%(84-87),但仍有33%需要进行活检来确定最终诊断。

综上,用较低的截止值排除晚期纤维化,而用较高的截止值排除肝硬化,这些标志物的特异性组合可以减少对肝脏活检的需求。
