对于使用他莫昔芬（TAM）治疗乳腺癌的女性，血栓栓塞是一种常见的不良事件。男性乳腺癌患者的风险很难调查。近日，肿瘤领域权威杂志British Journal of Cancer上发表了一篇研究文章，研究人员的目的是评估男性乳腺癌患者TAM治疗后血栓形成事件的风险。

在这项前瞻性队列研究中，研究人员评估了2009年5月至2017年7月期间448名接受治疗的男性乳腺癌（BC）患者，随访时间短于6个月的患者被排除在外。研究人员评估了受试者血栓栓塞的累积风险。

中位随访时间为47个月（范围为6-101个月），中位年龄为69.4岁（范围为27-89岁）。研究人员分别在98.3％和94.9％的病例中观察到雌激素受体和孕酮受体表达水平。在随访期间，接受TAM治疗的177名患者中有21名（11.9％）和未接受他莫昔芬治疗的41名患者中有1名（2.5％）患者发生血栓形成事件。估计发病率分别为每1000人每年51.9人和每1000人每年21.5人。值得注意的是，在最初的18个月中发现风险最高，其中发生了81％的血栓事件。年龄超过71岁的患者在TAM治疗后发生血栓形成事件的风险显著高于年轻患者（p=0.033）。血栓事件、心血管和肝脏疾病史以及额外的辅助治疗与血栓形成风险增加无关。

由此可见，男性乳腺癌患者在接受TAM治疗前18个月内发生血栓形成事件的风险显著增加，应在治疗决策中予以考虑。
