DECT材料分解的另一个应用涉及脂肪定量,主要用于检测高度流行的非酒精性脂肪肝中的肝脂肪变性。

自从新的快速kVp切换计算机断层扫描(CT)和双源CT扫描仪问世以来,基于源的双能量CT(DECT)技术已日益成为临床常规的一部分。由于DECT扫描仪提供的不同能量水平的测量信息,导致每种材料的能量具有特定的衰减系数,因此使得材料分解成为可能。双能量CT材料分解广泛应用于临床,其中提供灌注信息或产生虚拟无对比(VNC)图像碘定量是最常见评估最好的技术之一

DECT材料分解的另一个应用涉及脂肪定量,主要用于检测高度流行的非酒精性脂肪肝中的肝脂肪变性。非酒精性脂肪肝可发展为非酒精性脂肪性肝炎(NASH),是肝细胞癌的一个风险因素,是肝脏移植的第三大原因。与侵入性组织学、超声或耗时的磁共振波谱(MRS)相比,DECT的肝脏脂肪定量是客观、快速的,并能显示整个器官。

然而,尽管基于DECT扫描仪(如快速kVp开关CT或双源CT)脂肪定量研究很多,但在基于探测器dlsCT中脂肪定量材料分解迄今还没有被开发或评估。

近日,发表在INVEST RADIO杂志的一项研究为基于探测器的dlsCT脂肪定量开发了一项材料分解算法,并研究了其在患者肝脏和肌肉脂肪定量的适用性。

项可行性研究分别用0%、5%、10%、25%和40%的脂肪以及0、4.9和7.0毫克/毫升的碘建立了模型使用IQon光谱CT(飞利浦,荷兰)在120千伏和140千伏的条件下进行了模型扫描,并使用3T磁共振(飞利浦,荷兰)化学位移弛豫仪(MRR)和磁共振光谱仪(MRS)进行了扫描。基于光电效应和康普顿散射的maps对图像空间中的脂肪、碘和模型材料进行了3种材料的分解。纳入了10名需要进行CT分期的患者(平均年龄,55±18岁;6名男性)。所有患者都接受了120千伏的腹部对比增强dlsCT扫描和MRR的MR成像扫描。作为肝脏和骨骼肌的参考组织,使用了回顾性的非对比增强光谱CT数据集。通过类内相关系数(ICC)和Bland-Altman分析,评估了dlsCT和MR对每个病人的3个肝脏和2个肌肉感兴趣区域的一致性。 

在120千伏和140千伏的模型中,ICC非常好(dlsCT vs MRR 0.98 [95%置信区间(CI),0.94-0.99];dlsCT vs MRS 0.96 [95% CI,0.87-0.99]),在骨骼肌中(0.96 [95% CI,0.89-0.98])。对于对数转换的肝脏脂肪值,ICC是中等的(0.75 [95% CI, 0.48-0.88])。Bland-Altman分析得出,肝脏的平均差异为-0.7%(95%CI,-4.5至3.1),骨骼肌的平均差异为0.5%(95%CI,-4.3至5.3)。观察者之间和观察者内部的一致性都很好(>0.9)。

本项研究开发并建立了一个基于探测器dlsCT的脂肪定量,并成功地用于确定患者的肝脏和肌肉脂肪浓度。通过这种方法,可以在临床常规dlsCT扫描中检测到肝脏脂肪变性和肌肉质量下降这些具有预后意义的参数,为临床提供了一项新的影像学评估技术。
