近期发表的GLOBAL LEADERS研究2年随访结果显示，冠脉介入术前心率偏快是全因死亡的独立预测因素。
 
研究者发现，冠脉介入术前心率67次/分是预测2年死亡风险的最佳截断值。
 
冠脉介入术前心率>67次/分的患者术后2年全因死亡风险增加38%，心血管死亡风险增加64%，新发Q波心肌梗死的风险增加41%。
 
多变量Cox回归分析显示，冠脉介入术前心率每增快5次/分，全因死亡或新发Q波心肌梗死的风险增加6%，主要是全因死亡风险增加（HR=1.07）。
 
但冠脉介入术前心率与大出血（BARC 3型或5型出血）风险无关。
 
该研究还显示，冠脉介入术前心率偏快对患者预后的预测价值不受抗血小板治疗策略影响，也与患者是否为急性冠脉综合征或稳定性冠心病、是否存在糖尿病或肾功能受损无关。
 
研究者指出，理论上，应用β受体阻滞剂等减慢心率的药物可能会影响心率与临床预后之间的关系。
 
在该研究中，尽管没有记录基线β受体阻滞剂的应用情况，然而，校正出院时β受体阻滞剂、血管紧张素转换酶抑制剂、他汀等药物的应用后，心率仍然是2年全因死亡或新发Q波心肌梗死的独立危险因素。
 
Global Leaders研究是一项多中心的大型前瞻性随机对照优效性研究，由欧洲、北美、南美和亚太地区的130多家介入心脏病学中心参与。
 
这项最新分析一共纳入15 855例接受冠脉介入治疗的患者，其中位年龄为65岁，77%为男士，冠脉介入术前的心率中位数为69次/分。
 
与术前心率≤67次/分的患者相比，术前心率>67次/分的患者中女士较多，相对年轻，当前吸烟者比例、体重指数以及糖尿病、高血压、慢阻肺、急性冠脉综合征、既往中风、心梗、冠脉介入治疗、心脏搭桥术的比例均更高，左心室射血分数较低。
 
2年随访时，4.1%的患者死亡或新发Q波心肌梗死，术前心率>67次/分的患者中这一主要终点事件的发生率高于术前心率≤67次/分的患者（4.76% vs 3.31%）。

近期，Circulation刊登了接受冠状动脉介入治疗的心房颤动患者的抗栓白皮书。在评估口服抗凝药物和抗血小板治疗持续时间的关键临床试验结果后。专家组做出了重要更新，建议大多数患者应考虑在出院时采用双联抗栓方案（口服抗凝药加P2Y12抑制剂）。

BMJ：药物洗脱支架术后，6月双联抗血小板理想，>12月双抗大出血风险增加78%

对于冠脉介入术后的患者来说，双重抗血小板治疗（阿司匹林+氯吡格雷等P2Y12受体抑制剂）必不可少。尽管指南建议分别要接受至少12个月和6个月的双抗治疗，但近年来不断有研究对此提出了质疑，因为不少研究发现短期双抗可改善患者预后。近期，中南大学湘雅三医院心血管内科袁洪教授等人在《英国医学杂志》（BMJ）上发表的一项Meta分析证实，对于大多数置入药物洗脱支架的患者，双抗治疗≤6个月是最佳策略。
