标题：局部晚期非小细胞肺癌患者同步放化疗中紫杉醇每周一次用法vs.每周三次用法的一项随机化III期试验

背景：由于局部晚期非小细胞肺癌（LA NSCLC）的局部复发率较高，临床上对其进行最优管理仍具有挑战性。NCCN指南推荐把同步放化疗（CCRT）作为LA NSCLC的标准疗法。本III期试验研究的目的是：对比局部晚期非小细胞肺癌患者同步放化疗中紫杉醇每周一次用法（组2）与每周三次用法（组1）。

方法：在本多中心、随机化对照III期的试验中，接受诱导性化学疗法治疗2至4个周期后的LANSCLC患者被随机分配为两组。组1的化疗方案是紫杉醇（15mg/m2，3次/周，6周总剂量为270 mg/ m2）；在组2中，化疗方案是紫杉醇（45mg/m2，1次/周，6周总剂量为270 mg/ m2）的；放疗方案是60-70Gy（每次2Gy，每周5次）。

结果：2006年3月至2013年2月期间的115例患者被纳入研究。组1有63例患者，其中3例发生了3级放射性食管炎，3例发生了3级白细胞减少症，12例发生了3级放射性肺炎，1例发生了5级放射性肺炎、死于呼吸衰竭；组2有52例患者，其中5例发生了3级放射性食管炎，15例发生了3级或4级白细胞减少症，6例发生了3级放射性肺炎，2例发生了5级的放射性肺炎、死于呼吸衰竭。组1的反应率（87.3％）明显高于组2（57.7％）（P=0.023），而两组的PFS中位值分别为11.0ms和7.4ms（P =0.039）

结论：研究结果显示，LANSCLC患者CCRT中紫杉醇每周三次用法比每周一次用法更安全、有效。临床试验信息：ChiCTR-TRC-10000786。
