背景:吸入性布地奈德被认为是针对急性高山病新的预防用药。然而,其疗效还没有与急性高山病标准预防药物乙酰唑胺相比较。

方法:这项双盲、随机、安慰剂对照试验比较了吸入性布地奈德vs口服乙酰唑胺vs安慰剂,开始在早晨给药,在4小时内从1240m(4100 ft)上升至3810m(12570 ft)。主要结局是急性高山病发病率(头痛、路易斯湖调查问卷≥3和其他症状)。

结果:纳入了共103名参与者并完成了研究。33(32%)名接受布地奈德,35(34%)名接受乙酰唑胺,35(34%)名接受安慰剂。人口统计学资料无组间差异(P>0.09)。急性高山病的患病率是73%,严重急性高山病47%。与布地奈德组(n=24,73%)(比值比[OR],3.5;95%置信区间[CI],1.3-10.1)和安慰剂组(n=22,63%)(OR,0.5;95%CI,0.2-1.2)相比,乙酰唑胺组(n=15,43%)很少有参与者进展急性高山病。与布地奈德(n=18,55%)(OR,2.6;95%CI,1-7.2)和安慰剂(n=19,54%)(OR,0.4;95%CI,0.1-1)相比,乙酰唑胺(n=11,31%)可减少严重急性高山病。

结论:布地奈德对于急性高山病的预防是无效的,在快速上升之前服用乙酰唑胺可预防严重急性高山病。
