简要介绍：

根据第七次全国人口普查数据显示，我国60岁及以上人口为26 402万人，占比18.7%。伴随着人口老龄化加剧，老年慢性病具有发病独特、致残率高及疾病负担重等特点，已成为导致老年人失能的重要因素之一。有研究表明，20%～50%的不及时或延迟出院与缺乏恰当的出院准备服务(DP)相关,而DP不当或未标准化则可能导致不连续的护理，使病人出院后的护理风险增加。 基于最新循证证据并结合临床实践经验， 经多次函询和现场专家论证后， 起草了《老年患者出院准备服务专家共识( 2019 版) 》( 以下简称专家共识) ， 该共识属于国内首次提出的关于老年病人 DP 的临床指导意见。
