PLoS Med:新技术帮助鉴别恶性乳腺癌细胞显成效

发表于PLoS Medicine上的一项最新研究显示,利用一种分析动物和植物的生态学检测方法就可以对复杂多样化的乳腺癌进行鉴定,尤其是那些进展性及致死性的乳腺癌;这种检测方法可以用于临床中来评估乳腺癌患者疾病的恶性程度,并且帮助制定合适的疗法。

来自英国伦敦癌症研究院(The Institute of Cancer Research)的研究人员开发的这种名为生态系统多样性指数(Ecosystem Diversity Index)分析的检测技术可以将生态学家常用的方法同癌症成像技术结合来将癌细胞同正常细胞区分。该技术可以鉴别出极其恶性的乳腺癌亚型,而且相比其它疾病标志物而言该技术同时也是一种癌症患者生存的预测子,其可以帮助有效预测高级别癌症患者的生存几率。

文章中研究者对来自三所医院未经治疗的乳腺癌患者机体的1026份肿瘤样本进行分析检测,研究者分析了三种不同的细胞类型,即癌细胞、淋巴细胞及间质细胞。研究者指出,机体肿瘤尺寸大于5厘米的高级别肿瘤患者获得的生态系统多样性指数评分也较高,而且这种患者5年的存活率仅为16%,而低多样性指数评分的患者的5年存活率为50%。随后研究人员还观察了遗传因素对生态系统多样性指数的影响,这也是研究者首次分析肿瘤及遗传信息对不同细胞类型的影响。

他们发现,携带突变的p53肿瘤抑制子的高多样性评分的肿瘤患者的预后生存几率非常低,研究者Yinyin Yuan说道,我们的研究发现,生态多样性的数学模型可以帮助鉴别出恶性癌症,基于达尔文自然选择原则,通过分析肿瘤周围环境的图像,我们就可以更加有效地预测某些乳腺癌患者的生存,这要比如今在临床中的预测率要高很多。

长期以来我们知道癌细胞间的遗传多样性和疾病的恶性程度直接相关,而本文研究结果表明,肿瘤微环境内部的细胞多样性同样也可以促成恶性乳腺癌的发生;而将遗传改变的多个新型的测定结果进行整合就可以提供额外的患者预后信息,这或许为开发新型的诊断生物标志物提供了一定思路。

最后研究者Paul Workman说道,本文研究中研究人员巧妙地将生态学和细胞生物学技术相结合,开发了一种新型的预测检测技术,其将对临床中癌症患者的治疗带来很大帮助;我们始终会记得癌细胞并不会单兵作战,多个癌细胞会互相联合来生长发育,而更好地理解细胞间的微生态系统将会助力科学家们开发新型诊断、监测及治疗癌症的新策略。

这个研究给大家的启示是,临床上可以将不同的诊断因素进行整合,以达到最佳的诊疗效果。如影像学、组织病理学、基因组学、或一些血清biomarker,可以通过统计分析得出更具有高敏感性和特异性的新型“组合指标”。这是国际上大趋势。例如,在影像学中,除钼靶,超声之外,有很多新的方法,如双能CT,组织灌注CT,MR有很多新序列(如DWI,DTI等,可能用于乳腺分析)。而在血清标志物,组织标志物方面,也有很多新的内容,如CTC,外泌体技术,循环ctDNA检测等;在分子层面,各种基因标志物,但是每一种检测手段都不是完美的,但综合不同的手段,可能取长补短。
