做一次八九千的PET/CT有啥优势

很多人不知道PET/CT是什么,也分不清楚CT、MRI的区别是什么,更不清楚PET/CT在肿瘤诊断上的优势,今天给大家简单的聊聊这些相关的小知识

从原理上讲:

PET/CT是将放射性的药物(FDG,即葡萄糖类似物)静脉注入被检者的体内,经血液循环运送至全身,并通过细胞表面的葡萄糖转运受体(GLUT)被细胞吸收,然后放射性药物发射射线,通过机器采集射线信息,从而反映放射性药物的分布,功能越活跃,摄取FDG越多,在图像上则越浓;利用低剂量CT进行解剖定位,实现功能显像与解剖定位的有机结合。肿瘤细胞代谢活跃,能吸收更多的FDG,在影像上表现为放射性浓聚影,肿瘤组织功能改变可能较形态改变早,因此PET/CT检查能实现肿瘤的早期筛查

CT则是利用设备发射X线,穿过被检者的组织结构时发生吸收衰减,由探测器接收衰减后的射线并转换成电信号,再经模拟/数字转换成数字化影像。由于人体各组织器官密度的不同,在探测器上形成不同密度的组织影。

MRI,也就是大家常说的磁共振,是对某种磁场中的人体施加特定的射频脉冲,与人体内的氢质子产生共振,停止发射脉冲后,氢质子产生信号,被接收线圈接收,再利用梯度磁场进行空间定位,最后进行图像重建形成可见的影像学图像。

那么PET/CT对于肿瘤诊断的优势有哪些呢?

(1)PET/CT对肿瘤的早期诊断具有很大的优势。人类的很多疾病在早期会先出现功能上的异常,PET提供病灶详尽的功能与代谢等分子信息,具有灵敏、准确、特异及定位精确等特点。此时进行PET/CT检查,一次显像可获得全身各方位的断层图像,了解全身整体状况,达到早期发现病灶、诊断疾病和治疗疾病的目的。CT和MRI则是病变导致解剖结构发生改变时才能明确病灶,且两者均为局部显像,CT的诊断剂量较高。若患者体内装有支架,起搏器等金属部件,是MRI检查的禁忌,行CT检查时会产生伪影,影像诊断。

(2)PET/CT为全身显像,恶性肿瘤通常伴其他部位的转移,PET/CT能更全面观察全身转移病灶的情况,帮助肿瘤分期及临床决策。

(3)PET/CT对于监测治疗效果及监测肿瘤复发有明显优势。以淋巴瘤患者为例,化疗后,患者的病灶大小可能没有发生变化,但病灶的代谢活性已明显减低,PET/CT评估提示化疗有效,而普通影像学检查可能无法给出结论;另外对于肿瘤患者,治疗后最常面临的问题就是肿瘤复发,PET/CT可以在形态学发生改变前即发现转移病灶,较普通影像学更早,且更全面;对于消化道肿瘤患者,PET/CT较其他影像学检查能更好地检测是否发生肝转移,且检出率高。

(4)对于不明原因发热患者,PET/CT可以帮助寻找病因;PET/CT作为一种新的影像检查技术,较X线、CT、MRI等传统影像学检查方法在不明原因发热患者的诊断中敏感性更高,能尽早明确病因,作出诊断,缩短治疗时间,降低治疗成本。

最后划重点:PET/CT在肿瘤的早期筛查诊断、观察病灶转移、监测疗效、查找发热病因等方面具有优势。
