JASN:肾功能改变术后肌钙蛋白T水平与死亡率之间的关系

方法:
测量3天术后的心肌肌钙蛋白T浓度,若峰值超过0.02 ng/ml,则被认为是异常。

结果:
在这14037个病例中,267(1.9%)例术后30天内死亡。调整后的异常心肌肌钙蛋白T浓度的死亡风险率与透析速度分别为4.37,(95%置信区间,3.21到6.22),肾小球率过滤(eGFR)≥60 ml/min;6.15(95%置信区间,2.95到140.9),肾小球率过滤(eGFR)45~60 ml/min;6.30 (95% 置信区间,3.12 到21.23),肾小球率过滤(eGFR)30 ~45 ml/min;1.33 (95%置信区间, 0.56到4.85), 肾小球率过滤(eGFR)15 ~30 ml/min;1.46 (95%置信区间, 0.46 到 9.21),肾小球率过滤(eGFR)<15 ml/min每1.73 m2。

结论:
与患者每1.73 m2eGFR≥60ml/min相比,调整后的风险比每1.73 m2肾小球率过滤(eGFR)15 到30ml/min显著降低(P=0.02)。当重新定义异常cTnT的浓度,即大于等于0.03 ng/ml或者大于等于0.02 ng/ml时,并不能改变结果。而与术后肌钙蛋白T的水平相关的风险,当病人每1.73 m2肾小球率过滤(eGFR)小于30 ml/min时是不同的。另外本研究需要解释围手术期肌钙蛋白值低的患者肾功能不全的原因。
