Autophagy: “神药”二甲双胍可提升肌纤维病变中的肌肉功能

肌纤维肌病是一组在细胞水平上以蛋白聚集堆积和肌纤维结构衰竭为特征的慢性肌肉疾病。这类疾病的临床表现有很大的差异性,发病年龄跨度可以从婴儿一直到七十多岁。

肌纤维肌病是一组在细胞水平上以蛋白聚集堆积和肌纤维结构衰竭为特征的慢性肌肉疾病。这类疾病的临床表现有很大的差异性,发病年龄跨度可以从婴儿一直到七十多岁,肌无力从骨骼肌、心肌、呼吸肌的轻度减退到重度损伤,导致早期死亡。

共伴侣蛋白BAG3的显性突变是导致肌纤维肌病的原因之一,患者表现出进行性肌肉无力、肌肉结构衰竭和蛋白聚集。

最近,为了阐明BAG3肌纤维肌病的疾病机制,并确定治疗方法,研究人员构建了两种斑马鱼模型,一种是条件性表达BAG3P209L的模型,一种是bag3无义突变的模型。研究人员发现,转基因BAG3P209L表达的鱼显示出了蛋白质的聚集,可以模拟疾病的早期阶段,而bag3 -/-斑马鱼模型表现出运动依赖性纤维解体,并具有游泳活动能力的缺陷,与疾病的后期阶段一致。

对bag3 -/-鱼的详细表征,揭示了巨自噬/自噬活性的损害,研究人员也进一步在bag3患者样本中证实了这一缺陷。

综上所述,这些数据强调,虽然BAG3P209L的表达足以促进蛋白质聚集,但是由于BAG3在聚集物内的封存,导致自噬活性受损,以及随后的肌肉无力。

因此,研究人员随后对自噬促进化合物进行了筛选,以确定其在清除蛋白聚集物方面的有效性,并发现了包括二甲双胍在内的9种化合物。进一步的评估表明,二甲双胍不仅能够帮助清除斑马鱼和人类肌细胞中的蛋白质聚集物,而且能够拯救在bag3 -/-鱼中观察到的纤维解体和游泳缺陷。

因此,二甲双胍的再利用为BAG3肌病提供了一种有前景的治疗方法。
