Br J Dermatol :特应性皮炎患儿β-葡萄糖脑苷酶活性和角质层葡萄糖胆固醇水平升高与局部细胞因子环境的关系研究

本文测量了非皮损特应性皮炎患者皮肤在局部皮质类固醇治疗前后的β-葡萄糖脑苷酶(GBA)活性,发现其你指标均较前升高,提示GBA活性或GlcChol可能是监测AD治疗反应的生物标志物

      特应性皮炎(AD)是一种高度流行的炎症性皮肤病,其免疫反应和皮肤屏障功能异常广泛,皮肤屏障功能主要存在于皮肤的最上层,角质层(SC)由无核细胞(角质细胞)组成,角质细胞内充满角蛋白细丝,角蛋白细丝由微丝蛋白排列和聚集。角膜细胞被脂膜包裹,脂质层的锚定部位填充细胞间。高度有序的脂膜主要由神经酰胺(CER)、游离脂肪酸(FFA)和胆固醇(Chol)三类脂质组成。脂膜成分和结构的改变被认为是AD屏障缺陷的主要原因。此外,SC脂质,特别是CER是细胞信号中的关键分子,有助于调节重要的细胞功能。考虑到皮肤屏障的关键重要性,皮肤高度专门化CER的生物合成也就不足为奇了,这包括由许多酶介导的多个途径。几项研究表明,AD中与脂肪酸(FA)延长有关的酶的表达或活性发生了变化,这与皮肤屏障功能降低有关。此外,最近的一项研究报告,AD患者皮肤具有。一种酸性的β-葡萄糖脑苷酶(Gba),介导其前体葡萄糖神经酰胺(GlcCER)形成CER。在SC中,Gba是主要的葡萄糖苷酶。除了在GlcCER形成CER中的作用外,Gba还可以催化GlcCer和胆固醇之间的葡萄糖转移反应,从而形成葡萄糖胆固醇(GlcChol)。

    AD患者的GlcChol水平尚未被调查。GBA的表达或活性降低与通透性屏障降低有关,而遗传的GBS缺陷会导致严重的威胁生命的皮肤异常,如溶酶体储存障碍高谢病。此外,皮肤屏障的破坏会导致表皮GBA mRNA水平的增加。皮肤屏障破坏是AD(AD)的一个标志,它可能通过诱导GBA的表达来促进CER的合成,使皮肤屏障功能正常化。

    在这项研究中,我们的目的是定量测量非皮损AD皮肤在局部皮质类固醇治疗前后的GBA活性,并将其与健康皮肤进行比较。除了GBA活性,我们还测量了SC中GBA的两种酶产物,羟化神经酰胺(CER[H])和葡萄糖胆固醇(GlcChol),以及GBA的底物GlcCER[H]。为了探讨GBA活性和GlcChol水平是否与免疫反应、炎症和皮肤屏障功能有关,我们比较了疾病严重程度、经表皮水分损失(TEWL)和一系列已知在AD中起重要作用的免疫介质。

   方法收集健康儿童SC和AD患儿临床对照皮肤,分别于激素治疗前和治疗6周后测定脂质标志物和天然免疫细胞因子Th-1、Th-2,并与正常儿童对照,比较两组治疗前后血清中Th-1、Th-2、Th-1和Th-2的变化。用SCORAD评定AD严重程度,TEWL评定皮肤屏障功能。

   结果:基础GBA活性和GlcChol水平在AD患儿中升高,但在治疗后下降。AD组CER[H]和CER[H]/GlcCER[H]比值升高。GBA活性和GlcChol与TEWL和多种细胞因子水平相关,尤其是IL-1α和IL-18水平。GlcChol与疾病严重程度密切相关。

    我们发现AD患者GBA活性和GlcChol水平升高。数据表明炎症在紊乱的脂质处理中起着重要作用。GBA活性或GlcChol可能是监测AD治疗反应的有用生物标志物。
