有关维生素D水平与高血压的关系一直存在争议,例如:Nat Rev Cardiol:补充维生素D对治疗收缩性高血压无益 ,但新的研究证据,提供了更可靠的证据。发表在最新一期《柳叶刀—糖尿病和内分泌学》(Lancet Diabetes & Endocrinology)上该项研究提示,补充维生素D能够有效消除部分高血压的发病因素。

该项研究的负责人、南澳大利亚利亚大学的Elina Hyppönen教授提到,考虑到降压药物的费用和副作用,维生素D可能预防或降低血压及其风险因素的潜能引人关注。

维生素D在高血压病中扮演的角色一直受到关注,但截至目前,仍然没有证据表明其直接关系。观察性研究的结果已经提示了维生素D水平低下和血压增高及高血压的强相关性,但随即对照试验并未能提供一致证据。

这项利用D-CarDia协作基因数据的孟德尔随机化研究,包含了超过146500例源自欧洲血统的欧洲和北美个体样本。研究者利用两个常见基因变量,血清中的25羟基维生素D(25(OH)D)及其浓度(一般用于确认某一个体的维生素D状况),以测量维生 素D水平和血压或高血压风险间的因果效应。

在D-CarDia数据,表型分析中发现(n=49 363),血清25(OH)D浓度与收缩压下降有关,25(OH)D浓度每升高10%,可引起收缩压下降0.12mmHg(95% CI −0·20 to −0·04; p=0·003),减少高血压的风险2%( OR=0.98, 95% CI 0·97—0·99; p=0·0003),但对舒张压没有影响。采用meta分析,合并D-CarDia数据和以往35个研究共108173例样本时(合计146 581例样本(排除重叠样本),血清25(OH)D浓度与收缩压下降有关,25(OH)D浓度每升高10%,可引起收缩压下降0.1mmHg(−0·21 to −0·0001; p=0·0498),舒张压下降0.08 mmHg(−0·15 to −0·02; p=0·01)减少高血压的风险2%( OR=0.98, 95% CI 0·97—0·99; p=0·0003)

如果将D-CarDia数据与 consortia数据合并,进行meta分析发现(n=142 255),每个allele能下降高血压风险2%(OR per allele, 0·98, 0·96—0·99; p=0·001)。

进一步采用工具变量分析,25(OH)D浓度每升高10%,可引起舒张压下降0.29mmHg(−0·52 to −0·07; p=0·01),收缩压下降 0.37mmHg (−0·73 to 0·003; p=0·052),发生高血压的风险下降8.1%(OR 0·92, 0·87—0·97; p=0·002)。

研究者发现,25(OH)D浓度每升高10%,可引起舒张压下降0.29mmHg,收缩压下降 0.37mmHg,发展为高血压的几率则降低8.1%。

相关评论:

按照Hyppönen教授的观点,利用孟德尔随机化,能消除遗传多态性之间的差异,因此更有助于确定因果关系,可以更好地避免混杂因素、混淆因果及偏倚。然而,因为无法排除上述发现是偶然原因的可能,尚需独立的类似研究重复证明。也需要进一步的随机对照实验以确认补充维生素D的因果关系和潜在临床收益。

在随后评论中,来自丹麦哥本哈根大学和哥本哈根大学医院的Shoaib Afzal和Børge Nordestgaard博士写到,虽然该 项研究在弄清维生素D浓度低下和高血压发病机制的关系方面迈出重要的一步,但很多问题尚未可知。需要更多对这些结论的独立确认类似研究,为预防高血压引起 的如卒中等疾病的临床获益提供证据。最后,还需要随机干预试验来决定在诸如可被临床使用的策略前的维生素D补充给药法是否可以用来预防或治疗高血压。
