摘要:

依鲁替尼是Bruton酪氨酸激酶(BTK)一级抑制剂,在复发性/难治性(r/r)中枢神经系统淋巴瘤(CNSL)中表现出单药活性。但临床缓解多是一过性或不完全缓解,提示需要联合用药。

Christian Grommes等人开展一Ib1期临床试验,招募了15位CNSL患者,来探究依鲁替尼(560或840mg/日)联合甲氨蝶呤(HD-MTX)和利妥昔单抗用于CNSL患者的顺序组合。HD-MTX,每两周用药3.5g/m2,共8次(4个疗程,每疗程28天)。在予以HD-MTX的同时以及随后5天或HD-MTX代谢完后予以依鲁替尼。诱导疗法完成后持续监测依鲁替尼的单药活性,直到病程进展、不可耐受的毒性或死亡。

研究人员还对治疗前和治疗过程中脑脊液(CSF)中的环形肿瘤DNA(ctDNA)进行二代测序。依鲁替尼、HD-MTX和利妥昔单抗的联合方案耐受性良好,安全性佳(3级副反应4例,无5级副反应)。无剂量限制性毒性。

11位在完成4个周期的依鲁替尼/HD-MTX/利妥昔单抗联合化疗后进行依鲁替尼维持治疗。12位(80%)患者获得临床缓解。持续的肿瘤缓解与CSF中ctDNA的清除相关。
