  德国学者的一项研究表明,在可疑急性冠脉综合征患者中,入院至入院后3小时的高敏肌钙蛋白(hsTnI)和心肌肌钙蛋白I(cTnI)水平变化可能有助于早期诊断急性心肌梗死(AMI)。论文发表于《美国医学会杂志》[JAMA 201, 306(24):2684]。

  将高敏肌钙蛋白检测引入临床实践显著改善了胸痛患者的评估。此项研究共连续性纳入1818例可疑急性冠脉综合征患者。在入院时以及入院后3和6小时测定包括hsTnI(检测水平3.4 pg/ml)和cTnI(检测水平10 pg/ml)在内的12项生物标志物。主要转归为入院后3小时hsTnI和cTnI基线结果和系列变化对AMI的诊断性能。

  结果共有413例(22.7%)患者被诊断为AMI。对于AMI鉴别而言,入院时hsTnI和cTnI的受试者工作特征曲线(ROC)下面积分别为0.96和0.92。二者均优于其他诊断性生物标志物。入院时hsTnI的敏感性为82.3%,阴性预测值为94.7%。入院时cTnI的敏感性为79.4%,阴性预测值为94.0%。入院3小时后hsTnI和cTnI水平的敏感性和阴性预测值均为98.2%和99.4%。将入院时阈值与3小时内肌钙蛋白浓度系列变化相结合之后,hsTnI的阳性预测值由入院时的75.1%升高至3小时后的95.8%,cTnI则由80.9%升高至96.1%。
