血小板的下降,真的是药物惹的祸吗?

【案例经过】
2月17日晚临床联系自己,询问某患者血小板结果是否准确,血小板持续性下降有没有一些建议,患者血小板趋势如图1。

对于该患者还是有印象的,因为当时还推片染色了,油镜下血小板确实少见,符合仪器结果。而临床也比较疑惑:虽然患者最初病情重,血小板下降,考虑了重度感染因素,但随着患者状态开始好转,可以说感染被控制了一部分,血小板却还不见上升。患者状态由最初的昏迷、不能唤醒到现在的嗜睡、可唤醒能正确回答问题,血小板持续降低却未见明显淤血、瘀斑。

此时自己便犯了一个先入为主的错误,既然排除了其它因素导致的血小板下降,患者情况又在慢慢恢复,于是便联想到药物因素。查询医嘱发现舒普深(通用名:注射用头孢哌酮钠舒巴坦钠)贯穿着抗感染治疗。

2月18日临床查房示,血小板持续下降考虑重度感染,不排除药物引起可能。一切看起来似乎有点偏向药物引起血小板减少,但现实却重重的给了自己一巴掌。

【病例追踪】

在停用头孢哌酮钠舒巴坦钠及阿司匹林后,患者血小板依旧没有上升,当晚临床申请输注血小板预防治疗,2月19日复查血小板恢复至137×109/L。2月19日查房记录显示,患者神志清楚,能正确回答问题,随后患者由ICU转入普通病房。

【案例分析】

回顾整个案例,自己太过于执着关注血小板的下降这一方面,却没有很好的从整体考虑患者病情。事后,自己汇总患者PLT、CRP、D-D二聚体指标,如表1,发现随着时间的推移,血小板进行性下降,CRP与D-D二聚体均呈现先升高后下降趋势,如图2,CRP参考范围0~10mg/L,D-D二聚体参考范围0~0.6mg/L。

结合CRP居高不下说明患者体内确实存在感染,而感染是导致弥漫性血管内凝血(DIC)的主要原因之一,故有可能引发DIC的进程,从D-D二聚体的升高便可佐证,而恰恰DIC又能导致血小板消耗性减低。

那有没有可能患者存在DIC情况呢,根据中国DIC诊断积分系统(CDSS)发现该患者只能打到4分,小于诊断的7分标准(非血液病)。所以,该患者主要考虑感染导致的血小板减少,DIC诊断标准尚未达到。

有老师会问了,那其它因素(如药物)该如何排除呢?首先,我们要知道血小板的减少一般可从这四个方面着手考虑:血小板生成障碍或无效生成,血小板破坏增加或消耗过多,血小板分布异常,假性血小板减少症。

如果是药物导致骨髓抑制,进而引发血小板生成减少,那全血细胞如红细胞也应当减少,而该患者却没有。另外,血小板分布异常见于各种原因的脾大,导致脾脏可扣留全血85%的血小板,本案例患者并无脾大。镜检也可见血小板较少,未见聚集现象,故可排除假性血小板减少症。

所以,综合分析考虑该患者为血小板消耗增多导致PLT进行性下降,而消耗增多的“元凶”正是尚未完全被控制的感染。

【总  结】

此次血小板持续下降案例也告诉自己不可单纯仅看某一指标进行较真,而是应该结合患者所有检查及症状,从大局出发进行综合分析考虑。不可排除的是虽然有部分病例存在药源性导致血小板减少的案例,如马骁龙老师报道的《庆大霉素致药源性血小板减少症1例》,但药物引起的血小板减少毕竟还是较为少见,在工作中需谨慎做好排除性诊断。
