慢性肾脏疾病(CKD)患者常见贫血,这一现象归因于促红细胞生成素合成减少、绝对和功能性铁缺乏以及其他已知和未知的机制。随着CKD进展,贫血往往变得更为突出。在CKD G5阶段,尽管口服和静脉铁剂补充和促红细胞生成素类药物(ESAs)可用,但50%至80%的患者仍出现贫血。总体而言,CKD患者贫血与更糟糕的临床结果相关联。 钠-葡萄糖转运蛋白2(SGLT2)抑制剂可降低2型糖尿病(T2D)、心力衰竭和/或CKD患者的不良心血管和肾脏结局的风险。在其中一些临床试验中,与安慰剂相比,SGLT2抑制剂增加了血红蛋白浓度和红细胞比容水平。然而,仅有使用卡格列净治疗的T2D和CKD患者才显示了与贫血的临床相关性(根据研究者报告的贫血发生率或贫血治疗)。达格列净对CKD患者(无论是否合并T2D)的贫血矫正或预防效果尚不清楚。 在本篇DAPA-CKD(慢性肾脏病患者达格列净治疗试验)的事后分析中,我们评估了达格列净对红细胞比容、基线贫血患者的贫血矫正、基线无贫血患者的贫血发生率以及患有或不患有T2D的CKD患者的与贫血相关的不良事件的影响。

DAPA-CKD是一项多中心、双盲、随机、安慰剂对照试验,旨在评估达格列净对CKD患者的肾脏、心血管、死亡和安全结果的影响。试验招募符合条件的年龄在18岁及以上的患者,无论是否患有T2D,估算肾小球滤过率(eGFR)在25到75 ml/min/1.73 m²之间,尿白蛋白/肌酐比值(UACR)在200到5000 mg/g之间。此外,患者还需要稳定使用血管紧张素转换酶抑制剂或血管紧张素受体阻滞剂,除非有医学上的禁忌症。排除了患有1型糖尿病、多囊肾病、红斑狼疮性肾炎或抗中性粒细胞胞浆抗体相关性血管炎的患者。

参与者在21个国家的386个中心进行招募,招募时间从2017年2月2日至2020年6月12日,并随机分配到达格列净10 mg每日一次或相应安慰剂的1:1比例组中。根据T2D的存在和UACR(≤1000或>1000 mg/g)进行分层随机化。完整的试验方案已在先前描述过。DAPA-CKD试验按照《赫尔辛基宣言》的原则进行(ClinicalTrials.gov编号NCT03036150)。该试验方案已获得各参与机构的地方伦理委员会的批准。所有试验参与者均提供了书面知情同意。

血红蛋白比容测量在基线、2周、2个月和每4个月后进行。血红蛋白浓度仅在基线和最后一次研究访视时测量。所有测量均在中央实验室进行。

DAPA-CKD试验的预先指定的主要结果是eGFR持续下降50%或更多、末期肾脏疾病或肾脏或心血管原因的死亡的综合结果。重要的次要结果包括肾脏综合结果(不包括心血管死亡的主要结果)、心力衰竭住院或心血管死亡的心血管综合结果以及全因死亡。预定义的不良事件包括容量不足、肾脏事件、重度低血糖、骨折、截肢和潜在的酮症酸中毒。

与贫血相关的结果中,男性血红蛋白比容小于39%,女性小于36%被定义为贫血。我们将贫血矫正定义为基线贫血患者连续两次血红蛋白比容值超过这些阈值。相反,发生性贫血被定义为基线无贫血患者连续两次血红蛋白比容值低于这些阈值。在敏感性分析中,基线贫血使用血红蛋白阈值(男性<130 g/l,女性<120 g/l)来定义。当MedDRA首选术语中包含“贫血”一词时,药物中断或严重不良事件被认为与贫血相关。我们在附加分析中使用了从基线开始的绝对3.0%或更高的血红蛋白比容增加。3.0%或更高的血红蛋白比容增加被认为具有临床意义;相应的1.0 g/dl或更高的血红蛋白增加已被用作矫正贫血的临床试验的替代结果。

患者特征: 共有4304名参与者的基线平均年龄为61.8岁;其中1425人(33.1%)为女性,2906人(67.5%)患有T2D。基线时,4292人(99.7%)的血红蛋白比容数据是可用的。平均血红蛋白比容为38.9%±5.4%,1716人(40.0%)被定义为血红蛋白缺乏性贫血(表1)。与没有贫血的患者相比,贫血患者更可能患有T2D,eGFR较低,UACR较高。在贫血亚组中,达格列净组和安慰剂组之间的基线特征在两组间是平衡的。

达格列净相关的血红蛋白比容和血红蛋白结果: 与接受安慰剂的患者相比,接受达格列净随机分组的患者在第一次随机分组后的访视时(第2周)具有更高的平均血红蛋白比容(图1A)。两组之间的差异逐渐增加,从随机分组后的约4个月开始趋于稳定,并在整个试验期间保持在大约相同的水平(绝对两组间最小平方均差随时间变化,为2.3个百分点;95% CI,2.1至2.5)。达格列净治疗对血红蛋白比容的影响在预先指定的亚组中是一致的(图2),并且在治疗期间分析中的结果也相似(图S3)。最后一次研究访视时,达格列净组的平均血红蛋白水平为132.8±20.7 g/l,安慰剂组为125.1±19.4 g/l(图1B)。

达格列净相关的其他与贫血相关的测量结果: 在基线时患有贫血的患者中(图S1),达格列净组中有443名患者(53.3%),而安慰剂组中有247名患者(29.4%)(风险比为2.29;95% CI,1.96至2.68)达到与贫血矫正定义一致的血红蛋白比容水平(图3A)。在基线时没有贫血的患者中,与安慰剂相比,达格列净治疗与发生性贫血的风险降低相关(131人(10.4%)与300人(23.7%);风险比为0.39;95% CI,0.31至0.48)(图3B)。在基线亚组中,与贫血相关的结果显示出类似的关联性(图4),对于死亡的竞争风险(图3),或者根据血红蛋白浓度来定义基线贫血(图S2)。与贫血相关的不良事件的患者比例为达格列净组的47人(2.2%)与安慰剂组的82人(3.8%)(图3C)。在接受达格列净的1429名患者中,观察到3.0%或更高的血红蛋白比容增加,而在接受安慰剂的772名患者中,这个比例为37%(风险比为2.57;95% CI,2.35至2.81)(图3D)。

基线贫血与试验的主要和次要终点以及安全结果的关联: 在中位随访时间为2.4年的过程中,基线时有贫血的患者中有293人(17.1%),而无贫血的患者中有215人(8.3%)发生了主要复合终点(表S4)。在基线时患有贫血的患者中,次要终点和安全结果也呈现出类似的发现(表S4和S5)。在调整了基线危险因素(模型3)后,相对于无贫血患者,与患有贫血的患者相比,主要复合终点的风险比为1.71(95% CI,1.42至2.05),次要肾脏复合终点的风险比为1.91(95% CI,1.54至2.37),次要心血管复合终点的风险比为1.39(95% CI,1.07至1.82),全因死亡次要终点的风险比为1.46(95% CI,1.13至1.87)(表S4)。

基线贫血状态下达格列净对试验的主要和次要终点以及安全结果的影响: 当考虑贫血的相对尺度时,达格列净对主要和次要终点的影响在基线贫血亚组中是一致的。当考虑贫血的绝对尺度时,图5显示了相应的数据。对于主要复合终点和次要复合肾脏终点,观察到一定的异质性;在复合心血管终点和全因死亡终点上没有观察到异质性。使用血红蛋白阈值定义贫血时,重复这些分析得到了类似的结果。

本研究存在几个局限性。与贫血相关的不良事件没有进行裁决,而是基于研究者的报告进行定义。由于缺乏连续的血红蛋白浓度数据,我们使用了血细胞比容阈值来定义贫血,尽管使用基线和研究结束时的血红蛋白浓度定义贫血进行重复分析得到了定性相似的结果。我们无法评估达格列净治疗与口服或静脉铁剂、EPO治疗或输血之间的关系。最后,我们没有获得转铁蛋白饱和度、铁蛋白水平、促红细胞生成素或肝素蛋白的血清浓度,或其他可能影响我们对血细胞比容和离散结果数据解释的炎症指标。

总之,在这项对DAPA-CKD试验数据的事后分析中,我们发现达格列净治疗与血细胞比容增加相关,在基线时患有贫血的患者中纠正了贫血,并且在基线时没有贫血的患者中贫血的风险较低。
