JAHA：急性心肌梗死患者住院和出院后心力衰竭对预后有啥影响？

由此可见，HF发生，作为AMI的早期或晚期并发症，对患者的生存有负面影响。过度死亡率的变化与HF之间的相关性是因为与那些没有HF的患者相比，发生HF的AMI患者生存率适度改善有关。

心力衰竭（HF）是急性心肌梗死（AMI）的严重并发症。近日，心血管疾病领域权威杂志JAHA上发表了一篇研究文章，研究人员旨在探究作为AMI早期或晚期并发症的HF与随着时间变化的过量死亡率之间的相关性。

研究人员纳入了所有住院治疗的以往无HF住院史的AMI患者，并且在AMI出院后随访1年，观察HF发生情况。新发HF事件被分为住院HF（如果HF诊断是在AMI住院期间）或出院后HF（如果HF诊断是在AMI发病出院后1年期间内）。Logistic回归模型和Cox回归模型被用来探究过度死亡率与HF类别之间的相关性。研究人员通过检测HF状态与研究年份之间的相互作用来评估过度死亡率随着时间的推移的变化情况。

住院HF的发生使住院死亡率增加了1.79倍（比值比[OR]为1.79；95%可信区间[CI]为1.68-1.91）。从2001年至2009年，与HF相关的过度死亡率增加了4.3倍（P相互作用<0.001），主要是由于相比于住院HF（3%每年）患者，住院期间没有发生HF的患者（9%每年）住院死亡率下降更明显。出院后HF使全因和心血管疾病死亡率分别增加了5.98倍（风险比为5.98；95%可信区间为5.39-6.64）和7.93倍（亚风险比为7.93；95%可信区间为6.84-9.19）。相对过度的1年死亡率与HF之间的相关性并没有随着时间的推移而显著变化。

由此可见，HF发生，作为AMI的早期或晚期并发症，对患者的生存有负面影响。过度死亡率的变化与HF之间的相关性是因为与那些没有HF的患者相比，发生HF的AMI患者生存率适度改善有关。
