背景:妊娠期和哺乳期的叶酸需求在增加。建议备孕、怀孕或正在哺乳的女性摄入含叶酸(FA)的补充剂。

目的:旨在入组到MIREC(产妇婴儿环境化学物质的研究)研究的加拿大母亲队列中,确定母乳中总叶酸和未代谢叶酸(UMFA)的含量,及其与FA补充剂的使用和剂量之间的相关性。

设计:使用液相质谱联的方法检测母乳四氢叶酸(THF)、5-甲基-THF、5-甲酰基-THF、5、10-甲叉亚甲基-THF和UMFA(n=561)。每日总补充FA的摄入基于自我报告的FA补充剂的使用。

结果:96.1%女性的乳汁中可检测到UMFA。每天从补充剂中FA的总摄入与乳汁叶酸浓度和种类相关。补充剂使用者(401人)比不使用者(160人)的乳汁总叶酸浓度高18%(P<0.001),该差异由摄入>400μg FA/d的女性造成(P≤0.004)。补充剂使用者比不使用者的5-甲基-THF低19%(P<0.001),UMFA高126%(P<0.001)。摄入>400μg FA/d的女性比摄入≤400μg FA/d的女性的5-甲基-THF相应降低,UMFA相应升高。

结论:FA补充剂的使用与乳汁总叶酸适度增加相关。可检测到的乳汁UMFA几乎普遍存在,包括未摄入FA补充剂的女性。摄入>400μg FA/d的女性,其乳汁UMFA相应的比5-甲基-THF高,因此表明,较高的剂量超过了FA的生理代谢能力,造成母乳中FA优先摄取。因此,FA补充剂的剂量>400μg并不推荐,尤其是强制性强化补充FA的人群。
