OMCL:益肾排毒方通过抑制小鼠氧化应激、炎症和上皮-间充质转化起到对糖尿病肾损伤的保护作用

糖尿病肾病(DKD)是糖尿病最常见的慢性微血管并发症之一,但目前尚缺乏有效的治疗策略。益肾排毒方(YSPDF)是一种中药制剂,临床用于治疗慢性肾脏病(CKD)已有20多年的历史。

目的:糖尿病肾病(DKD)是糖尿病最常见的慢性微血管并发症之一,但目前尚缺乏有效的治疗策略。益肾排毒方(YSPDF)是一种中药制剂,临床用于治疗慢性肾脏病(CKD)已有20多年的历史。然而,YSPDF对DKD是否有治疗作用尚无研究。

方法:本研究通过建立高糖(HG-)诱导的肾损伤细胞模型,观察益肾排毒方对 2型糖尿病DKD模型db/db小鼠的影响,并揭示其作用机制。

结果:养肾排毒方能明显改善大鼠的各项生化指标(空腹血糖、血肌酐、血尿素氮、24h尿总蛋白、总胆固醇、总甘油三酯),改善肾组织病理改变和肾组织纤维化。此外,还能明显抑制氧化应激状态和炎性细胞因子(TNF-α、IL-6、IL-1β和单核细胞趋化蛋白-1)水平。养肾排毒方治疗可显著减轻肾纤维化,有证据表明这是通过抑制转化生长因子-β-1/Smad通路抑制上皮向间充质转化而实现的。有趣的是,服用YSPDF后,已知与氧化应激相关的蛋白Nrf2、HO-1和NQO1的表达显著增加YSPDF治疗组NLRP3炎症体蛋白NLRP3、ASC、caspase-1和裂解caspase-1水平降低。细胞实验表明,益肾排毒方能抑制HG暴露的HK-2细胞的EMT和NLRP3炎性小体,其机制可能与激活Nrf2有关

结论:我们的研究表明,养肾排毒方可能通过抑制氧化应激、炎症和EMT而减轻db/db小鼠的肾损伤,其机制可能与激活Nrf2途径有关。
