Eur Respir J：一般人群中肺动脉与主动脉比率和全因死亡率的风险有何关系？

由此可见，较大的PA：A与老年人群的死亡率无关，但是其是中重度COPD患者死亡率的独立决定因素。通过对其他适应症的CT患者测量PA：A可能对COPD患者提供了重要的预后信息。

肺动脉与主动脉比值（PA：A）＞1是肺动脉高压的一个指标。目前尚不清楚这项指标是否在一般人群中和慢性阻塞性肺病（COPD）患者中携带有预后信息。

近日，呼吸疾病领域权威杂志Eur Respir J上发表了一篇研究文章，研究人员在2003年至2006年,纳入了2197名来自鹿特丹的人群为基础的研究的参与者（平均年龄为69.7±6.7 岁；女性占51.3%），这些参与者接受了心脏计算机断层扫描（CT）对PA：A进行了量化，研究人员将其定义为肺动脉和主动脉的直径之间的比率。COPD的诊断是依据临床表现、肺功能以及治疗医生测量得到的阻塞性肺功能。研究人员采用Cox回归分析了死亡风险。

研究人员观察到PA：A每增加1个标准差与一般人群的死亡率之间没有相关性。更大的PA：A比值与COPD患者的死亡率增加相关，特别是中重度COPD患者（风险比为1.36，95%可信区间为1.03-1.79）。研究人员证明了COPD的死亡率风险是由严重的COPD所驱动，并且这种风险随着弥散能力的降低而增加。

由此可见，较大的PA：A与老年人群的死亡率无关，但是其是中重度COPD患者死亡率的独立决定因素。通过对其他适应症的CT患者测量PA：A可能对COPD患者提供了重要的预后信息。
