盘点:近期“摄入蛋白质”相关研究汇总

【1】多吃植物蛋白,有助于预防代谢综合征

研究纳入了5324例参与者,来自墨尔本合作队列研究,基线时均没有心血管疾病、癌症、高脂血症、血糖升高、血压升高、腰围升高(1990-1994)。使用经验证的121项食品频率问卷评估参与者的膳食摄入情况;在基线和随访时(2003-2007)检测参与者的MetS(代谢综合征)情况。

在平均11.2年随访时间里,发生了459例新发MetS。能量摄入来自总蛋白摄入的最高四分之一组与最低四分之一组比较,MetS的OR值为1.46 (1.01-2.10);能量摄入来自动物蛋白摄入的最高四分之一组与最低四分之一组比较,MetS的OR值为1.67 (1.13-2.48);能量摄入来自植物蛋白摄入的最高四分之一组与最低四分之一组比较,MetS的OR值为0.60 (0.37-0.97)。

MetS的发病率与以下蛋白质来源呈正相关:肌肉(OR 1.37 (1.00, 1.87))、红肉(1.47 (1.01, 2.15));MetS的发病率与以下蛋白质来源呈负相关:谷物(0.62 (0.40, 0.97))、豆类和坚果(0.74 (0.53, 1.04))。

超过11年的时间,来自动物蛋白的能量每增加5%,腰围增加0.97 cm (95% CI: 0.50, 1.45)、收缩压增加0.97 mmHg (95% CI: 0.13, 1.82)、体重增加0.94 kg (95% CI: 0.57, 1.32)。

不过动物蛋白摄入量与以下因素的变化呈负相关:腰围(-1.38 cm (95% CI: -2.68, -0.07))、体重(-1.97 kg (95% CI: -3.00, -0.94))。

【2】孕期过多摄入蛋白质可能对后代的生长不利

研究者对来自1961对母亲–孩子(来自Project Viva研究)的数据进行了分析,探究母亲在怀孕期间的蛋白质摄入量与后代线性增长之间的关系。

平均(范围)孕中期蛋白质摄入量为1.4克/每天每公斤体重(0.3–3.1)。校正母亲社会人口统计学资料、孕期体重增加、母亲和父亲身高、子女性别、胎龄、母乳喂养持续时间后,孕中期母亲蛋白质摄入量每增加1-SD (0.36克/每天每公斤体重),后代长度z分数变化为−0.10 (95% CI: −0.18, −0.03),从出生到6个月,长度增加斜率变化为−0.03厘米/月(95% CI: −0.05, −0.01),从6个月到7岁,该变化值为−0.09厘米/月(95% CI: −0.14, −0.05)。至于孕早期母亲蛋白质摄入量和后代长度增加的关系,与孕中期类似。

结果表明,对于一个孕期相对高蛋白质摄入的人群而言,更高的蛋白质摄入量与更短的新生儿出生长度有关,以及与更慢的线性生长速度有关(出生到7岁)。结果表明,在怀孕期间摄入更多的蛋白质,并不会增加胎儿和儿童的生长,甚至还会不利于后代早期长度生长。(文章详见——Am J Clin Nutr:孕期过多摄入蛋白质可能对后代的生长不利

【3】未患乳糜泻,不建议摄入无谷蛋白饮食

研究人员评估了2009-2014年NHANES研究中的部分数据,参与者共有22278名,且年龄均为6岁以上。此外,研究人员通过血液测试确立患者是否患有乳糜泻,通过访谈评估患者摄入无谷蛋白饮食的依从性。

共有106名(0.69%)参与者被诊断出患有乳糜泻,此外,213名(1.08%)参与者尽管未患有乳糜泻,但是仍然采取无谷蛋白饮食。

研究人员估计,美国约有176万人患有乳糜泻,270万人没有乳糜泻诊断却依旧采用无谷蛋白饮食。
研究人员发现6年来美国民众乳糜泻患病率保持稳定:2009-2010年患病率为0.70%,2011-2012年为0.77%,2013-2014年为0.58%。然而,坚持无谷蛋白饮食却无乳糜泻诊断的民众人数比例持续增加:2009-2010年为0.52%,2011-2012年为0.99%,2013-2014年为1.69%。

有研究证实,谷蛋白是导致民众患乳糜泻的风险因素之一,无谷蛋白饮食更廉价和方便,因此,很多人尽管不存在乳糜泻诊断,也会摄入无谷蛋白饮食。此外,很多人存在谷蛋白敏感性但尚不具有乳糜泻诊断,因此这些人也会采取无谷蛋白饮食。(文章详见——JAMA INTER MED:未患乳糜泻,不建议摄入无谷蛋白饮食

【4】植物蛋白代替动物蛋白,可降低死亡率

该研究共包括131342名来自护士健康研究和健康专业人员随访研究的参与者。动物蛋白的来源包括红肉、家禽、鱼、蛋和奶制品,而植物蛋白的来源包括面包、意大利面食、坚果、豆类和豆类。

数据显示,根据能量百分比进行评估,中位数动物蛋白的摄入量为14%,(第五至第九十五百分位,9%-22%);植物蛋白的摄入量为4%(第五至第九十五百分位,2%-6%)。

校正生活方式和饮食危险因素后,他们发现,动物蛋白摄入量与较高的死亡率有关,能量增量每增加10%,死亡风险增加1.08(95% CI, 1.01-1.16; P = .04)。植物蛋白摄入量与死亡率降低相关,能量增量每增加3%,死亡风险下降0.9(95% CI, 0.86-0.95, P < .001)。然而,这些研究结果仅限于含有不健康的生活方式的参与者,比如吸烟、久坐不动、肥胖/超重、酗酒等;对于具有健康生活方式的参与者而言,不存在以上联系。

【5】母乳中的蛋白可以降低早产儿感染风险

足月产的婴儿可以从母体中获得天生的保护防御系统,从而帮助他们避免感染的危险。但是早产儿缺乏保护性肠道细菌,抗感染能力不能有效的发展。最近在密苏里堪萨斯大学医学院的研究人员发现母乳中的一种乳铁蛋白可以帮助早产儿预防葡萄球菌感染。

该研究纳入了120例早产儿,婴儿的出生体重在在737g到1474g之间。60%的婴儿接受了为期28天的乳铁蛋白,在护理时一天两次通过仿真奶嘴喂养。

为了掌握蛋白质在保护性肠道细菌的发展时的作用,研究人员检测了婴儿的粪便。发现接受乳铁蛋白的新生儿体内负责葡萄球菌感染侵袭的病原被根除。

Sherman博士说:这些细菌是早产儿院内感染的最常见的病原菌,50%的感染由它们引起。另外,作为医生,关于乳铁蛋白如何影响肠道细菌的知识我们知道的还有限,但我们的研究表明它可以限制细菌并且保护早产儿避免葡萄球菌的感染。

作为研究的一部分,乳铁蛋白是免费提供给患儿,Sherman说:一剂量的乳铁蛋白需要大约25美元到500美元。感染可使婴儿住院10-14天需要花费40,000美元到56,000美元。

虽然现在推荐乳铁蛋白作为全国新生儿重症监护病房的标准治疗方案为时尚早,但研究人员指出已有更多的研究可以证实其预防感染的作用。

【6】通过高蛋白饮食减肥可改善睡眠质量

美国普渡大学的一项新研究表明,通过高蛋白质饮食减肥的超重和肥胖的成年人睡眠更好。

一项试验研究发现,在14名参与者,4周减肥后摄入更多的膳食蛋白质反而有了更好的睡眠。然后,在主要研究中,44名超重或肥胖的参与者被列入研究,摄入正常蛋白或更高蛋白质含量的减肥饮食。3周适应饮食后,在16周中各组每天摄入每公斤体重0.8或1.5kg的蛋白。在整个研究中,参与者完成了调查,以评估他们每月的睡眠质量。那些摄入更多的蛋白质减肥的参与者被报道睡眠质量在三到四个月饮食干预后有了改善。

营养师设计的饮食能够满足每个研究参与者的每日能量需要,保持每天脂肪和碳水化合物在750卡路里,基于在较高或正常蛋白组维持蛋白含量。在两项研究中蛋白都来源于牛肉,猪肉,大豆,豆类和牛奶中的蛋白。
