如果能够鉴定在疾病过程早期发生的慢性肾病(CKD),而且特异于肾功能丧失而不是其他潜在的疾病原因的标志物,可以更及时准确地确诊即将发展为CKD的患者。因此,本研究试图通过使用创新的反向孟德尔随机化MR)方法来鉴定由肾功能丧失引起的早期CKD诊断血液标志物。

研究人员将这项技术应用于4147名患者的基因和生物标志物数据,这些患者均患有已知的2糖尿病、空腹血糖受损或糖耐量受损。利用CKDGen Consortium (n = 133814)中与肌酐eGFR (eGFRcrea)相关的变异进行双样本MR,以评估eGFRcrea基因降低对238个血清生物标志物的影响。

研究发现反向MR和三叶因子3TFF3)为由于eGFRcrea降低而增加的蛋白质(β= 1.86SD / SD降低eGFRcrea; 95CI0.95-2.76; P = 8.0×10-5)。反向MR结果与ORIGINCKD事件的流行病学相关性一致(OR = 1.28TFF3每增加SD; 95CI1.18-1.38; P = 4.58×10-10)。加入TFF3后,相对于单独的eGFRcrea,显著提高了对CKD事件的辨别(净重新分类改善= 0.211; P = 9.56×10-12)。

研究结果表明,TFF3是血糖异常人群早期CKD的一种有价值的诊断标志物,并为应用这种新的MR技术可以为识别其他慢性疾病的诊断生物标志物提供参考。
