单发巨型肝癌根治性切除后无复发生存率和总生存率的新预测模型,可将SLH-HCC患者准确地分为术后低、中、高危复发和死亡风险组

肝细胞癌(HCC)是全球癌症相关死亡的主要原因。中国HCC的发病率和死亡率占全球的一半以上。由于HCC无明显症状,大多数HCC患者往往在晚期时才被诊断出来,部分患者确诊时HCC肿瘤直径已长到5.0 cm以上,甚至是10.0 cm(巨大肝癌)以上。

迄今为止,尚未建立无门静脉癌栓的单发巨型肝癌(SLH-HCC,直径>5.0 cm)术后的生存预后模型。因此,本研究旨在开发新的诺模图来预测这些患者术后的复发和生存情况。

研究人员回顾性的收集在2005年1月至2015年12月在中国进行根治性切除术的2469位SLH-HCC患者的数据。具体来说,采用训练队列(n=1012)的无复发生存期(RFS)和总生存期(OS)数据建立了模型,然后在内部验证队列(n=338)和外部验证队列(n=1119)中进行了验证。

研究诺模图与5种常规HCC分期系统预测能力的对比

在训练队列中,基于肿瘤临床病理特征(直径、分化、微血管浸润、甲胎蛋白)、手术因素(术前经导管动脉化疗栓塞治疗、肝切除范围及术中输血)、肝功能和系统炎症或免疫状态建立的PFS和OS的诺模图都取得了高C-指数,分别是0.85和0.86,明显高于5种常规HCC分期系统的预测(PFS和OS的分别是0.62-0.73和0.63-0.75)

根据诺模图评分分层的患者的RFS和OS:A、B、C:RFS;D、E、F:OS;A、D:训练队列;B、E:内部验证队列;C、F:外部验证队列

而且,该模型的表现在内部验证队列和外部验证队列中均得到了很好的证实(内部验证队列:RFS和OS分别是0.83和0.84;外部验证队列:PFS和OS分别是0.87和0.88),具有良好的拟合校准曲线。该模型可将SLH-HCC患者准确地分为术后低、中、高危复发和死亡风险组

综上,该研究建立的诺模图模型可很好的预测SLH-HCC患者根治性手术后的RFS和OS,有助于指导医生和患者及时进行预防干预。
