BMJ:肥胖患者减肥干预对死亡率、心血管疾病和癌症的影响!

由此可见,通常为低脂肪和低饱和脂肪的减肥饮食,无论是否进行运动干预,都可能减少肥胖成年人过早的全因死亡。

近日,顶级杂志BMJ上发表了一篇研究文章,研究人员旨在评估肥胖成人减肥干预是否会影响全因、心血管和癌症死亡率、心血管疾病、癌症和体重。

研究人员使用随机效应对随机对照试验(RCTs)进行系统评价和荟萃分析,估计了风险比和平均差异;使用Cochran的Q和I2统计量来调查研究的异质性。按照GRADE标准来评估证据质量。分析的数据来自于Medline、Embase、Cochrane对照试验中心登记数据库,以及试验登记处数据库中结果不明显的全文。研究人员联系了作者获取未发表的数据。选择随机对照研究(RCT)的标准为针对肥胖成年人体重减轻进行饮食干预,进行或未进行运动建议或项目,随访超过1年。

该荟萃分析共确定了54个随机对照试验,共涉及30206名参与者。除一项试验外,所有试验都评估了低脂肪、减肥饮食。对于主要结局,高质量的证据表明减肥干预降低了全因死亡率(34项试验,685次事件;风险比为0.82,95%可信区间为0.71-0.95),每1000名参与者中有6例死亡(95%可信区间为2-10)。对于其他主要终点,中等质量的证据显示其对心血管死亡率有效应(8项试验,134次事件;风险比为0.93,95%可信区间为0.67-1.31),而非常低质量的证据显示其对癌症死亡率有效果(8项试验,34次事件;风险比为0.58,95%可信区间为0.30-1.11)。24项试验(15176名参与者)报道了参与者新发心血管事件的高质量证据(1043次事件;风险比为0.93,95%可信区间为0.83-1.04)。19项研究(6330名参与者)提供了参与者新发癌症的非常低质量证据(103次事件;风险比为0.92,95%可信区间为0.63-1.36)。

由此可见,通常为低脂肪和低饱和脂肪的减肥饮食,无论是否进行运动干预,都可能减少肥胖成年人过早的全因死亡。
