标题:单核苷酸多态性与吸烟状况对非小细胞肺癌的预测

背景:肺癌已成为死亡率最高的恶性肿瘤。其5年生存率小于15%,但早期肺癌患者经过有效治疗其5年生存率可高达87.7%,由此可见,早期肺癌的及早诊断是影响肺癌患者生存率的关键。然而,当前肺癌的诊断水平并不能够提高早期肺癌的检出率。本研究的目的在于引入单核苷酸多态性(SNPs)遗传因素来构建肺癌的预测模型,以有助于提高早期肺癌患者的检出率。

方法:在最近的多个的研究中均显示了单核苷酸多态性同非小细胞肺癌具有关联。我们的研究召集了3637个中国受试者,其中包括1597例受试者及2040例对照者,用于对单核苷酸多态性进行再评估。多因素Logistic回归模型采用单核苷酸多态性,年龄,性别,有无吸烟状况作为预测因子用以预测非小细胞肺癌。使用受试者工作特征曲线(ROC)下的面积对预测诊断的准确性进行评价。

结果:结果显示,以6个单核苷酸多态性同吸烟状况及人口统计数值作为预测因子的Logistic回归模型的曲线下面积为0.63(95% CI: 0.61–0.65; P=1.6×10-38),高于以非遗传模型同吸烟状况及人口统计数值作为预测因子的曲线下面积0.60 (95% CI: 0.59–0.62; P=4.5×10-25)。在女性和具有吸烟史的组别中,以5个及4个单核苷酸多态性作为预测因子的曲线下面积分别为0.61(95% CI: 0.57–0.65;P=8.44×10-9)和0.57 (95% CI: 0.55–0.60; P=1.53×10-7)。在腺癌组,以3个单核苷酸多态性同性别作为预测因子曲线下面积为0.71(95% CI: 0.62–0.80; P=3.91×10-5),高于以非遗传模型同性别作为预测因子的曲线下面积0.57 (95% CI: 0.48–0.66; P=0.157)。

结论:单核苷酸多态性的组合有助于肺癌风险模型的诊断。本研究显示了整合单核苷酸多态性对于肺癌风险的预测模型是具有意义的。我们的研究结果也显示了性别,吸烟史及肺癌组织类型的分层分析对于提高肺癌的特定风险模型的诊断的重要性。
