指南制定组主席,芝加哥Ann and Robert H. Lurie儿童医院的病理学家Lawrence J. Jennings在一份声明中表示,在精准医学时代,NGS已经迅速成为检测多种体细胞变异、诊断疾病以及预测疗效的手段之一,然而分析验证过程仍然具有挑战性。

美国分子病理学学会召集了包括来自美国病理学会的多学科专家,共同总结了当前知识、揭露存在的挑战,并提供指导以最好的验证检测,最终改善患者的护理。

该指南特别强调了分子实验室对建立最佳NGS检测方法的重要性,并指出随着技术的前进,未来指南会不断更新。该指南强调了NGS检测的开发、优化和验证,包括panel内容的选择建议、验证前进行优化;使用参考细胞系和参考物质来评估检测性能;确定每种突变类型的阳性百分比和阳性预测值;明确最低覆盖深度以及最小样本量。指南还指出,在积累验证数据之前建立试验验证协议非常重要,并且应该使用所分析的样本类型进行验证。指南还提醒:检测性能会根据被检测的基因组区域而异,而且尽管使用最好的仪器或最专业的人员,检测结果还是会出现差异。

此外,实验室检测的定量分析性能(例如NGS检测),并不一定达到临床水平的预测性能。疾病的内在生物变异对临床敏感性以及NGS检测的特异性影响最大。专家小组建议在试验设计时就明确临床有效性和临床实用性。NGS在检测多种体细胞变异上具有很大的优势,但在使用过程中还存在着挑战,并且需要分析验证来保证高质量结果。

这份指南为进一步规范NGS在临床中的应用,提供了规范。
