Stroke:替奈普酶静脉溶栓治疗大血管闭塞患者的疗效分析

与接受静脉阿替普酶治疗的患者相比,接受替奈普酶静脉溶栓治疗的LVO急性缺血性卒中患者再通和临床效果显著提高。

来自越来越多的随机对照临床试验证据表明,替奈普酶可能替代阿替普酶治疗急性缺血性卒中的有效疗法。近日,血管疾病领域权威杂志Stroke上发表了一篇研究文章,在该系统评价和荟萃分析中,研究人员试图比较静脉替奈普酶与静脉给予阿替普酶治疗急性缺血性卒中大血管闭塞患者的疗效和安全性。

研究人员对MEDLINE和Scopus数据库进行了检索,查找已发表的随机对照临床试验,这些试验提供了急性缺血性卒中患者结局,并确诊为LVO,且接受不同剂量的替奈普酶或0.9 mg/kg阿替普酶标准剂量的静脉溶栓。该研究的主要结局是在3个月时改良Rankin量表评分为0到2的几率。

研究人员纳入了4项随机对照临床试验,共433例患者。接受替奈普酶治疗的LVO患者改良的Rankin量表评分为0至2(比值比为2.06 [95%CI为1.15-3.69])、再通成功(比值比为3.05 [95%CI为1.73-5.40])和功能改善(所有改良的Rankin Scale评分降低1分(常见比值比为1.84 [95%CI为1.18–2.87]))的可能性更高,相比于接受阿替普酶治疗的LVO患者。对于上述结局纳入的研究提供的结果之间几乎没有异质性(I2≤20%)。接受替奈普酶或阿替普酶静脉内溶栓治疗的LVO患者的早期神经功能改善、症状性颅内出血、任何颅内出血以及改良的Rankin Scale评分为0到1或3个月全因死亡率均无差异。

由此可见,与接受静脉阿替普酶治疗的患者相比,接受替奈普酶静脉溶栓治疗的LVO急性缺血性卒中患者再通和临床效果显著提高。
