世界首例3D打印椎体植入术在京完成

今年5月初的一天,身高1.8米的明浩和同学踢足球,一个简单的顶球动作竟使他的枢椎受了伤。顶球后,明浩并没有太大感觉。第二天他的脖子才开始疼,家人以为他落枕了。一个多月过去了,落枕没好,明浩全身开始麻木。在山东老家的医院检查后发现,明浩枢椎出现骨折。因为部位特殊、病情较重,当地医生建议到北京就诊。7月中旬,一家人辗转来到北医三院,找到了国内脊柱肿瘤专家——骨科主任刘忠军教授。经影像学检查,骨折原因高度怀疑枢椎部位肿瘤;经穿刺活检,确定肿瘤的性质——尤文氏肉瘤!

人的颈椎由七节椎骨组成,其中,最上面与颅底相连的两节分别为寰椎和枢椎,这两块椎体位置和形状都比较特别。尤文氏肉瘤是一种病情发展迅速、恶性程度高的溶骨性病变。枢椎部位的尤文氏肉瘤,随时可能出现不可逆的脊髓损伤,导致完全瘫痪、呼吸骤停。

刘忠军和团队经研究决定,为明浩的枢椎进行前路和后路两次手术。这种手术难度很高,其一,通过手术切除枢椎被称为手术禁区,全国能够完成枢椎切除手术的医院只有不过5家。其二,仅仅切除被肿瘤侵蚀的枢椎还不行,还要想办法在第一和第三颈椎之间添上植入物,帮助明浩的头抬起来。

传统手术切除病变椎体后,会用钛合金网笼代替原来椎体的位置,但容易出现椎体塌陷,椎间高度难以维持等情况。刘忠军团队尝试用钛合金粉末3D打印技术制造出人工椎体,植入后不再需要钛板辅助固定。

据介绍,医生将明浩的CT数据传给制造企业,工程人员再将数据通过软件分析重建三维立体图形。工程人员通过软件识别患者骨骼内的病变部位,提取此骨骼信息以及病变组织信息,以此为基础,进行假体设计。设计好后,技术人员将钛合金粉放入3D打印机的锻造箱内,依据假体的设计数据,打印出内植物。

7月31日,最关键的二期手术开始。5个小时的手术中,明浩病变的第二颈椎被切除,植入3D打印人工椎体;术后第5天,明浩从重症监护病房转到普通病房;术后12天,明浩可以戴着支具下地行走。这也是世界首例3D打印椎体植入术成功实施。
