MR弹性成像以及US弹性成像,作为儿童和成人弥漫性肝病的非侵入性检查手段,已在多项临床领域中获得了实质性的进展。弹性成像技术作为一种诊断和监测疾病的严重程度定量方法,可以提供常规影像学检查所无法提供的功能参数,肝脏硬度阈值就是其一。肝脏硬度阈值将健康肝脏与病变肝脏区分开来,为进一步的诊断及研究打下基础。然而,在儿童MR弹性成像中定义肝脏硬度阈值的文献仍然很少,且阈值各不相同。

已有研究表明,成人与儿童的肝脏硬度阈值存在差异。此外,先前的儿科研究结果的差异可能技术或MRI扫描仪供应商相关的场强以及MR弹性成像的驱动频率之间的差异有关

近日,发表在Radiology杂志的一项研究通过MR弹性成像供应商和场强测量,确定儿童正常肝脏硬度和相关的正常认同的范围,为临床建立规范的儿童肝脏硬度数据提供参考依据,为进一步优化儿童肝脏病变的诊断阈值开辟道路

本研究为一项前瞻性多中心队列研究。于2018年2月至2019年10月期间,在4个地点招募无已知肝脏病史的7-17.9岁志愿者,并对每位志愿者进行MRI扫描和血液检查。MRI在三种供应商平台和两种场强(1.5 T和3.0 T)下进行。所有MRI扫描均进行集中分析;硬度、质子密度脂肪分数(PDFF)和R2*值均使用平均值进行表示。计算肝脏硬度的平均和95%置信区间(CIs)。采用Pearson相关系数(r)、双样本t检验及方差分析评估单变量相关性。

最终共71名志愿者有完整的检查数据(中位数年龄,12岁;四分位数间隔[IQR],10-15岁;39名女性)。中位体质量指数百分比为54% (IQR, 32.5% ~ 69.5%)。肝脏硬度均值为2.1 kPa (95% CI: 2.0, 2.2 kPa),均值±1.96 kPa标准差为1.5-2.8 kPa。肝脏PDFF中位数为2.0% (IQR, 1.7%-2.6%)。肝脏硬度与所有患者变量或MRI扫描参数之间没有相关性

本研究发现无基础肝病的健康儿童的肝脏硬度与成人相似,为2.1 kPa,第95百分位为2.8 kPa。这与先前学者所提出的2.74 kPa阈值接近,因此这提示本研究提出了一个合理的区分健康和病变肝脏的阈值,这为临床对小儿肝病的进一步诊断及研究提供了理论基础。同时本研究还发现,健康儿童的肝脏硬度与性别、年龄或体重指数无关,且在不同的MRI扫描仪供应商及场强之间不存在统计学差异,这也为肝脏硬度的标准化测量提供了技术方面的参考意见。
