Sci Adv:治疗阿兹海默病,电脑有望立奇功

最近,来自英国剑桥大学的科学们发表了一种创新性的寻找单抗药物的方法——针对患者脑内聚积的β淀粉样蛋白,完全依靠电脑设计单抗药物。通过这一方法获得的单抗药物在体外实验和线虫模型实验中取得了良好的效果,这一研究发表在了最近的《科学》子刊《Science Advances》上。

阿兹海默病由于其临床试验成功率极低而被称之为新药研发的“坟墓”。但是,由于该病影响范围很大,使得研究机构和生物制药企业继续在这方面加大投入。针对阿兹海默病患者脑内的有毒蛋白聚积物的单克隆抗体药物是目前的研究热点之一,这类药物在临床试验中遭遇过失败,但近期也有一些积极的早期数据出炉。 最近,来自英国剑桥大学的科学们发表了一种创新性的寻找单抗药物的方法——针对患者脑内聚积的β淀粉样蛋白,完全依靠电脑设计单抗药物。通过这一方法获得的单抗药物在体外实验和线虫模型实验中取得了良好的效果,这一研究发表在了最近的《科学》子刊《Science Advances》上。

阿兹海默病是全球影响最广的老年失智症,总共有超过5000万名患者。由于它的病程很长,给患者家庭和社会都会带来很大的经济负担。由于其病因至今不明,也没有有效治疗的药物,仅有一些可以延缓症状发展的药物。从病理学的角度来看,阿兹海默病患者的脑部存在着大量由于蛋白不正常折叠而导致的沉积,这些蛋白沉积主要来自与β淀粉样蛋白和tau蛋白。在蛋白聚积的过程中,还会产生一种毒性非常高的寡聚物。寡聚物和蛋白沉积都会影响神经细胞的正常功能,甚至导致其死亡,从而造成患者的认知能力衰退。

在过去几十年中,科研人员花费了大量的努力来阻止或减少这些蛋白沉积的产生,但是,到目前为止仍然没有能够改变疾病进程的药物出现。免疫疗法是最近十几年来兴起的一种非常具有潜力的手段,它具有非常高的特异性。对于阿兹海默病来说,可以使用选择性针对β淀粉样蛋白或tau蛋白的疫苗或单抗药物,以激活人体的免疫系统,像对待外来入侵物一样对待有毒蛋白沉积并将其清除出大脑。

现有的开发单抗药物的方法主要有两种。第一种是有超过50年历史的经典方法,即将抗原如β淀粉样蛋白注射到动物体内,动物的免疫系统会作出反应产生针对这个抗原的抗体,通过筛选可以找到其中特异性较高的可成药单抗。第二种方法是上世纪90年代出现的不需要动物的方法,通过在实验室产生的大型抗体库中寻找针对特定抗原的单抗药物。这两种方法都费时费力,并且可能无法筛选到最为理想的候选药物。

随着计算机技术的日益强大,以及结构生物学的发展,使得用电脑从头设计一个抗体成为可能。电脑设计不仅节省了大量的时间和资源,更重要的是,它不仅能针对一个特定的抗原设计抗体,它还能够针对这个抗原上的特定位点设计有针对性的抗体。这使得科研人员能够针对β淀粉样蛋白聚积过程中的特定机制以及在其中起关键作用的特定位点。此外,电脑设计产生的单结构域抗体比传统单抗药物更小,因此更容易穿过血脑屏障,并且更不容易引起系统性的免疫反应,从而降低副作用。

剑桥大学的科学家们并没有直接针对β淀粉样蛋白设计抗体,他们的目标是聚积的β淀粉样蛋白。他们发现了在β淀粉样蛋白沉积中暴露在外的5段长约7-8个氨基酸的短肽序列,针对这5个短肽,分别设计了高亲和度的氨基酸序列并得到了5个单结构域抗体。这5个抗体与β淀粉样蛋白本身的亲和度并不高,但是却能高度亲和β淀粉样蛋白的寡聚物或沉积。

随后进行的体外实验发现,这个5个抗体中有2个能够非常有效地阻止β淀粉样蛋白聚积过程中的两个不同机制,一是由β淀粉样蛋白单体聚合形成寡聚物的初级成核过程,二是在已经形成的β淀粉样蛋白表面再形成寡聚物的次级成核过程。科研人员认为,这两种机制分别主要出现在早期和中后期阿兹海默病患者中,因此这两个抗体分别针对早期和中后期患者。

为了进一步验证这两个抗体的作用,科研人员在秀丽线虫模型中进行了实验。过量表达β淀粉样蛋白的线虫会出现严重的运动功能障碍甚至瘫痪,不同年龄的线虫大致对应了不同阶段的阿兹海默病进展。实验发现,在1天或3天大的线虫中使用第一个早期抑制抗体,以及在6天大的线虫中使用第二个中后期抑制抗体,都能够挽救线虫的运动功能障碍。

这项研究的第一作者Francesco Aprile博士因为本研究被阿兹海默病学会授予了2017年度“失智症研究早期职业生涯杰出贡献奖”。阿兹海默病学会研究部门主管James Pickett博士评论道:“Aprile博士及其同事采取的创新方法通过先进的计算机技术来设计专门阻止疾病关键进程的药物,从一个新的角度来开发阿兹海默病药物。近50年来,抗体技术的进步为类风湿性关节炎、多发性硬化症和某些癌症提供了新疗法。我们对于这项早期工作的潜力感到兴奋,希望能为阿兹海默病也带来抗体药物。”
