European Radiology:深度学习在自动检测肺结节中的应用

深度学习系统(DLS)作为一项新兴的人工智能技术,可直接从数据中学习影像特征,越来越被人们所熟识及使用。

随着人们健康意识的逐渐增强在计算机断层扫描(CT)上检测肺部结节成为放射科医生一项重要而又费时的工作。原发性肺癌的发病率最高,是癌症中的主要死因,早期发现仍然是提高肺癌患者生存率的最佳方法。全身其他部位的癌症也容易扩散到肺部,早期发现肺部转移对优化治疗选择至关重要。

深度学习系统(DLS)作为一项新兴的人工智能技术直接从数据中学习影像特征,越来越被人们所熟识及使用。随着大数据集的出现和计算能力的提高,DLS在从胸部CT图像中检测肺部结节方面取得了初步成功,但其范围仅限于有固定图像协议的肺癌筛查项目。在日常工作中,如何将DLS与CT结合起来进行更广泛的应用成为一项人们迫切需要解决的问题

近日,发表在European Radiology杂志的一项研究评估自动检测肺部结节的深度学习系统(DLS)作为影像科医生辅助诊断价值

项单中心回顾性研究筛选了2018年9月至2019年2月的21150份连续CT扫描。在轴位CT图像上标记由DLS检测到但在最初的放射学报告中未提及的肺部结节。被标记的图像由四位获得委员会认证的放射科医生评分,每位医生至少有5年的经验。然后,根据2017年Fleischner肺结节指南,将得分为2(可以理解的错过)或3(不应错过)的结节分为不可能有临床意义(2a或3a)或可能有临床意义(2b或3b)。漏报率被定义为获得2或3分的图像总数除以筛选的图像总数。 

在DLS标记的172个结节中,60个(35%)漏检结节被放射科医生确认。这些结节分别在24、14、10和12项研究中被进一步归类为2a、2b、3a和3b,总体阳性预测值为35%。在所有CT图像中,有0.3%发现了遗漏的肺部结节,其中三分之一的病变被认为具有临床意义。


 遗漏的肺结节示例,50岁男性,有肝细胞癌(HCC)治疗后定期随访的病史史。深度学习系统(DLS)在左肺上叶检测到一个7毫米的磨玻璃密度灶(GGO)(a;放大图 b)。在3个月的随访中,发现这个GGO的密度有所增加(c)。患者接受了手术,组织病理学结果为原位腺癌。患者接受了切除手术,HCC或肺癌均未复发。在DLS的提醒下,治疗方法有所改变。

使用DLS辅助的自动检测系统进行辅助诊断,可以发现需对具有临床意义的被遗漏的肺部结节。因此临床上有必要进一步收集数据和改进反馈系统,以确定早期肺癌检测的经济效益,加强放射科医生对肺结节的检测。
