J Endod：上颌**磨牙尖周健康与上颌窦粘膜增厚之间的关系

这篇回顾性研究的目的是为了评估上颌窦粘膜增厚(MSs)与健康和患病上颌牙齿牙周以及解剖因素的X线影像特征之间可能存在的联系。

研究通过CBCT-尖周索引评估50名患者(mean age = 40.60±14.59 years)上颌磨牙的尖周健康情况。评估上颌磨牙与MSs下壁之间的解剖关系以及牙槽骨缺失与MS发病之间的联系。评估这些可能的影响因素对MSs粘膜增厚的作用。采用Kruskal-Wallis， Mann-Whitney U， chi-square和logistic regression (for relative risk)检验对结果进行统计学分析。

结果显示，CBCT-尖周索引4是发生根尖周炎(AP)损伤最常见的内容。存在AP时，MSs发生病理变化的风险明显增高(P < .001)。这种风险是同一患者的MSs 毗邻健康牙齿的62.364倍(95% confidence interval， 7.968-488.14)。当AP磨牙根尖靠近MSs时，MS粘膜明显增厚(P < .004)。牙周状态和MS病理状态之间未见显著性差异。

结论：这篇研究的结果显示，MSs会受到临近磨牙牙髓健康状态的影响。在检查MS病理变化时，口腔及内科医生应该考虑牙源性致病的可能。

上颌第二磨牙根管上端弯曲伴MB21例

根管弯曲，特别是根管入口处弯曲可增加根管治疗的难度。上颌第二磨牙髓室及根管系统的变异率更高，狭窄的髓室底视野较差，易导致MB2根管的遗漏，同时也增加了预备MB2的难度。1－2型MB2是根管上段为1个根管口，中下端分为两个根管，其发生率约3.7%。我们接诊了1例上颌第二磨牙根管入口急弯伴近颊根1－2型的患者，现报道如下。

牛牙症是指牙齿的牙体长而牙根短小，髓室顶至髓室底的高度高于正常，根分叉向根尖处移位，且颈溢线不明显甚至消失。此种牙似有蹄类动物牙，故称为牛牙样牙或牛型牙，发生率0.61%～11.30%，通常认为是由牙齿发育过程中Hertwig＇s上皮根鞘内陷延迟所引起。C型根管是一种变异的根管系统，以牙根的横断面呈C型为特征，下颌第二磨牙最常见。临床上，按照Melton分型，C型根管常规分为三型：C1型为根管口连

右侧上颌**磨牙MB2根管1例

患者女，27岁。因右上后牙咬合疼痛不适数日前来就诊，5年前曾于外院行牙髓治疗。检查：16牙合面大面积龋损、髓腔暴露，探（－），叩（+），不松动，16、17颊侧黏膜见窦道（图1）；17残冠，髓腔暴露，根管口可见牙胶样充填物，叩（－），不松动，16、17临床牙冠短。

患者男性，20岁，因左上中切牙牙龈处反复脓包就诊。自述曾于约10年前有过一次篮球撞击伤，曾于外院治疗但无明显好转。专科检查：21牙冠形态正常，可见21上方约5mm牙龈处一窦道开口，挤压有脓性液体渗出，21未见明显牙体组织疾患，唇侧可探及6mm深窄牙周袋，腭侧10mm深窄牙周袋，电测无活力，叩痛+，I～II度松动。X线片：21根尖区低密度影，根管内高密度影像，根管影像不清晰（图1）。初步诊断：21慢

患者男，46岁，汉族。因左上颌前牙咬合痛1月余就诊。检查：反牙合，牙结石（++），全口牙龈肿胀，22近中大面积龋损，牙冠外形异常，舌隆突高起，叩痛，冷热刺激痛，无松动。X线片示：22牙冠异常，主根管腔被牙中牙分割为颊舌向的双根管，根管内有似釉质、牙本质密度影像，中央有一空腔，形态似一小牙；根尖发育基本完成，根尖周有大面积稀疏区。
