Hypertension:血脂异常与先兆子痫风险

全世界有5%的孕妇患有子痫前期并发症,研究人员观察到HDL-C升高对子痫前期风险具保护性作用。

全世界有5%的孕妇患有先兆子痫并发症,在美国有4%,且发病率不断上升,是每年孕产妇和婴儿死亡的主要原因之一。但先兆子痫在国家之间和国家内部存在着相当大的差异,撒哈拉以南非洲国家的疾病负担最高,而在美国,非西班牙裔黑人妇女的风险最高。由于疾病负担高,人们对降低子痫前期的风险和确定风险分层和潜在的治疗目标有很大的兴趣。在观察性研究中,血脂异常与子痫前期有关。研究人员使用孟德尔随机化分析来估计4个组的血脂水平、药理靶标和子痫前期风险之间的关系。

近日,心血管权威杂志Hypertension上发表了一篇研究文章,研究人员从欧洲、混合非洲、拉丁美洲和东亚血统参与者的全基因组关联研究中提取了与低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)和甘油三酯密切相关(P<5×10-8)的单核苷酸多态性。先兆子痫风险的遗传关联是从同一群体的研究中提取出来的。

在Meta分析之前,研究人员对每个组分别进行逆方差加权分析,并进行了敏感性分析以评估由于遗传多效性、人口学统计指标和间接遗传效应引起的偏倚。

跨4个组的荟萃分析纳入了150万名有血脂指标的受试者,7425名受试者伴有先兆子痫,239290名受试者没有先兆子痫。HDL-C升高与先兆子痫风险降低相关(HDL-C每增加一个SD的比值比为0.84 [95%CI为0.74-0.94];P=0.004),这在敏感性分析中是一致的。研究人员还观察到胆固醇酯转移蛋白抑制(一种增加HDL-C的药物靶点)可能具有保护作用。研究人员观察到LDL-C或甘油三酯对子痫前期风险的影响不一致。

由此可见,研究人员观察到HDL-C升高对先兆子痫风险具保护性作用。研究人员的发现与LDL-C修饰药物在试验中的结果不一致,但表明HDL-C可能是筛选和干预的新目标。
