JAMA:羟乙基淀粉 vs 生理盐水对腹部术后高肾损伤风险患者预后的影响

研究认为,对于腹部大手术且术后肾损伤风险高的患者中, 0.9%生理盐水与羟乙基淀粉的胶体溶液进行容积置换治疗,对患者死亡或术后主要并发症的影响无显著差异

近日研究人员比较了含有羟乙基淀粉的胶体溶液(HES130/0.4)与0.9%生理盐水对腹部大手术后高危患者死亡率和术后并发症的影响。

775名成人患者在2016年2月至2018年7月在法国20所大学医院接受腹部大手术,患者术后肾损伤风险增加。患者随机接受含有6% HES130 / 0.4的0.9% 生理盐水(n389)或仅0.9% 生理盐水 (n386),每次250毫升。研究的主要结果是术后14天死亡或主要术后并发症。次要结果包括术后14天内预先确定的术后并发症、重症监护室和住院时间,以及术后28天和90天的全因死亡率。

826名患者参与研究,平均年龄68岁,91名女性,775名(94%)患者完成了试验。HES组389例患者中的139例(36%)出现主要终点事件,而生理盐水组386例患者中125例(32%)出现终点事件(差异为3.3%;相对风险为1.10)。在12项预先指定的次要结果中,11项无显著性差异。术后28天,HES组和生理盐水组分别有4.1%和2.3%的患者死亡(差异为1.8%;相对风险为1.76)。

研究认为,对于腹部大手术且术后肾损伤风险高的患者中, 0.9%生理盐水与羟乙基淀粉的胶体溶液进行容积置换治疗,对患者死亡或术后主要并发症的影响无显著差异。
