Eur Heart J:多基因风险评分预测房颤的效能

在有心血管疾病的患者中,AFPRS是AF发生的一个强有力的独立预测因子,当添加经过验证的临床风险评分和NT-proBNP时,它提供了互补的预测价值。

人们对心房颤动的定向筛查项目越来越感兴趣,但遗传学在识别心房颤动高危患者中的作用尚不清楚。近日,心血管领域权威杂志Eur Heart J上发表了一篇研究文章,四项TIMI试验共分析了36662名无心房颤动的受试者。研究人员通过验证的心房颤动多基因风险评分(PRS)五分位数将受试者分为五组,并使用CHARGE-AF模型计算心房颤动的临床风险。此外,研究人员使用Kaplan-Meier事件发生率、调整后的风险比(HRs)、c指数和净重分类改进来确定添加PRS与临床风险和b型利钠肽前体n端(NT-proBNP)相比是否能改善心房颤动的预测效能。

在2.3年随访期间,有1018例新发心房颤动病例。AFPRS预测AF风险呈现梯度,PRS每增加1-SD,AF的风险增加40%[HR为1.40(1.32-1.49);P<0.001)。与心房颤动低PRS(房颤最低20%)受试者相比,房颤高低PRS(房颤最高20%)受试者发生房颤的风险增高2倍多[HR为2.45(1.99-3.03),P<0.001]。

此外,PRS在CHARGE-AF临床风险评分的基础上提供了额外的风险分层,范围从低临床和遗传风险患者的3年发病率1.3%到高临床和遗传风险患者的8.7%。高临床风险、高PRS和NT-proBNP升高的患者在3年内发生房颤的风险为16.7%。单独使用CHARGE-AF临床风险评分的c指数为0.65,加入NT-proBNP后提高至0.67(P<0.001),加入PRS后进一步提高至0.70(P<0.001)。

由此可见,在有心血管疾病的患者中,AFPRS是AF发生的一个强有力的独立预测因子,当添加经过验证的临床风险评分和NT-proBNP时,它提供了互补的预测价值。
