基因编辑2.0:基因突变疾病迎来精准修复

像CRISPR这一类的使用核酸酶的基因编辑方法,因为涉及到剪切 DNA 双链结构,可能有一定几率导致不精准的基因编辑,所以近来也受到了一些质疑。而某些不会破坏 DNA 双链结构的基因编辑方法也都不乏研究者,比如腺相关病毒(AAV),以及下面要提到的“碱基编辑”。

在今天分别发表于《Science》和《Nature》的两项研究中,来自麻省理工学院和哈佛大学 Broad 研究所的科学家们描述了一种编辑 DNA 和 RNA 的方法,他们称之为“碱基编辑”(base editing)——这种基因编辑方法将有可能用于一系列目前还没办法彻底治愈的遗传疾病。

这种方法之所以有效,是因为其实有时候 DNA 长链中只会有某一对碱基发生变异,这种现象被称为“点突变(point mutation)在已知的与疾病相关的 5 万种人类基因变异中,有 32000 种是由点突变造成的。

在发表于Nature的论文中,由哈佛大学化学教授、Broad 研究所研究员 David Ruchien Liu 领导的团队成功将单碱基 A 转换为 G,这种调整可能解决已知的 16000 种与点突变相关的疾病。

为了达到目的,研究团队对现有 CRISPR 技术做了修改,使其靶向特定的单碱基。新的基因编辑工具可以重构单碱基 A 的原子结构,使其重组为 G,从而触发细胞对其他 DNA 链进行修复以完成整个转换过程。而最终的结果就是 A-T 碱基对被成功转换为 G-C 碱基对。这种技术的精妙之处就在于,将基因编码中的错误进行重写,而不是像传统编辑方法那样对 DNA 片段整体进行剪辑或切除。

“标准的基因编辑方法,比如 CRISPR-Cas9,可能会对 DNA 双链结构造成破坏。但如果目标是要插入或敲除某些 DNA 碱基,这种方式还是非常有效的。”David Liu 在本周二进行的媒体电话会议上表示。“但如果目标仅仅是修复某个点突变,碱基编辑无疑是一种更安全有效的方式。”

David Liu 教授还表示,新的碱基编辑方法并不是要替代传统的 CRISPR 基因编辑技术,而是为治疗某些疾病提供其他可选的方案。打个很容易懂的比方:如果说 CRISPR 是一把剪刀,那碱基编辑则更像一支铅笔


谈到基因编辑的安全性及可替代方法,DT 君此前对著名遗传生物学家 George Church 教授进行专访时,他也表达过类似的观点:“我完全赞同这些方式。我多年来一直在讲,即便是在 CRISPR 出来之前,其实就有比核酸酶更好的基因编辑方法。CRISPR 只是切断 DNA 双链的方法之一,其他的还包括巨核酸酶等方法。但这类核酸酶的缺点是会解旋 DNA 双链,有一定几率导致不精准的基因编辑。”

对于基因编辑的可替代方法,Goerge Church 教授表示:“我在很多场合都表示过我们应该开拓思路,寻找更多的基因编辑替代方法。比如你提到的 AAV,这类方法与核酸酶相比,好处在于无需破坏 DNA 双链结构。我很看好这些替代方法,而且我本人也确实在从事这方面的研究。”

其实在此之前,研究人员就已经创造出了碱基编辑工具,可以把 G 转换为 A,有 15% 的疾病相关突变涉及到这种转换。在今年九月份,中国中山大学的研究人员就成功使用碱基编辑工具移除了胚胎中会导致地中海贫血的遗传突变。

David Liu 和他的同事们从病人身上采集细胞,然后使用他们创造的碱基编辑工具修正会引起遗传性血色素沉着症的点突变。这种疾病最大的问题是会导致身体从食物中吸收过多的铁,而随着时间的推移,身体内的铁越积越多,严重的话会引发糖尿病、心脏病、关节病甚至肝癌和其他肝脏疾病。

David Liu 和他的团队还使用碱基编辑方法来抑制人体细胞内镰状细胞性贫血症(Sickle-Cell Anemia)。在这两项研究中,碱基编辑完全没有脱靶现象以及额外的 DNA 插入或缺失,而这正是传统CRISPR的缺陷。


而在今天的《Science》上还刊登了另一篇重磅论文,同样是 Broad 研究所的张锋教授使用了一种相似的碱基编辑方法来对 RNA 中的腺嘌呤(A)进行单碱基编辑,而由于 RNA 在人体中会自然降解,所以这一编辑的过程不会导致人类基因组的永久性改变,同时还有望精准地矫正会导致疾病的遗传突变。

加州大学伯克利分校创新基因组学研究所(Innovative Genomics Institute)的罗斯·威尔逊(Ross Wilson)表示,碱基编辑最终可能会成为治疗某些疾病更好的方法。他打了一个形象的比方,在碱基编辑治疗中,一个单一的碱基对就像一段文字中的一个字,可以十分精准地进行修改,而如果使用传统的 CRISPR 技术,就不得不更换整个段落,二者效果的差别高下立判。 

David Liu 说,他希望 DNA 和 RNA 的碱基编辑可以用作“现有的大量潜在治疗手段”的补充疗法。他的实验室目前正在探索使用碱基编辑来治疗血液疾病、神经障碍、遗传性耳聋和遗传性失明等疾病。
