单次大剂量口服维生素D可以有效减轻由紫外线引起的皮肤损伤。

炎炎夏日已经来临,一时疏忽忘记涂防晒霜,要怎么补救才能有效防止晒伤呢?Scott等人最近研究表明,摄入维生素D可能比涂抹芦荟胶更有效。适度暴露在紫外光下可以让我们的皮肤合成维生素D前体,并在肝脏和肾脏中羟化成为活化型。一直以来,维生素D被认为是维持体内钙稳态和保证骨骼健康的重要因素。尽管最近它也被认为可以降低癌症和心血管疾病的风险,降低传染病和自身免疫疾病的易感风险,目前仍没有足够的证据足以推荐其在骨骼相关疾病之外的口服用药。

为了检测维生素D是否能够减轻由紫外线引起的皮肤炎症反应,Scott等人初步展开了一项随机双盲的研究。在接受一定量紫外光暴露的1小时后,20个成年被试随机口服三种不同剂量的维生素D或安慰剂。与安慰剂组和低剂量实验组相比,200,000国际单位的维生素D摄入可以明显减轻晒后48小时的皮肤损伤和泛红症状。其保护作用与两种主要的紫外线引起的炎症介质——介导肿瘤坏死因子α和诱导型-氧化氮和酶(iNOS)在皮肤中的表达减少相关。基于全部皮肤样本的微阵列芯片结果的非监督式层级聚类显示,一些基因显著聚集在了减少炎症介质表达和皮肤屏障修复通路中。我们将这种摄入高剂量维生素D3而减轻皮肤泛红和高血清药物浓度的现象称为“维生素D响应”。

这项研究结果显示,单次大剂量口服维生素D可以有效减轻由紫外线引起的皮肤损伤。尽管研究人员们已经注意到,可能引起诱导型-氧化氮和酶(iNOS)减少的1型精氨酸酶在“维生素D响应”过程中上调表达,与维生素D相关的皮肤炎症保护机制依然不明确。即使这项法相在大量的研究中得到重复,我们依然需要明确高剂量维生素D是否会通过对紫外线损伤和免疫介导的组织修复及受损细胞的去除的解耦连,而导致突变率升高。目前为止,皮肤科医生仍然建议坚持涂抹防晒霜以避免晒伤。
