Heart:心肌梗死亚型分类的现状!

此项研究结果显示L:总体而言,本分类中有一个适度的观察者间一致性,κ系数(κ)为0.55。区分1型心肌梗死有明显的一致性(κ:0.61),而区分2型心肌梗死(κ:0.54)仅有中度一致性。在多因素logistic回归分析中,ST段抬高心肌梗死(P <0.001),冠状动脉造影(P <0.001)和肌钙蛋白水平较大变化(P = 0.023)均独立使得医生同意的次数更多,而他们对患者的症状,如呼吸困难(P <0.001),收缩压较高(P = 0.001),入院时C反应蛋白水平较高(P = 0.016)更多的表现为不同意。

此项研究结果表明:鉴别MI的类型对经过培训的内科医生也具有挑战性。尽管严格遵守MI的第三个通用定义,但是1型心肌梗死,2型心肌梗死和心肌损伤之间的区别只能提供适度的观察者间一致性。迫切需要更精确和临床适用的现行分类标准,特别是2型心梗诊断标准。
