第二代PD-1抗体药物M7824寻求国内上市,肿瘤免疫治疗一片红海

目前已有的临床研究数据显示该药物较其他抗癌药物(包括其他PD-(L)1药物)有着巨大的优势,因此市场前景很被业界看好。值得一提的是,刚在我国上市的Opdivo(O药)和Keytruda(K药)在国内上市一个月的销售额就分别达到了1.9亿和1.5亿元,因此,疗效更佳的M7824如果成功上市,必将快速成为肿瘤界的重磅药物。

1、药物开发人员为何开发二代PD-1

免疫检查点抑制剂失效率太高!这就是最大的理由。

研究显示,包括肺癌乳腺癌结肠癌、黑色素瘤和淋巴瘤等在内的众多癌症都是通过刺激机体免疫抑制,从而逃脱免疫监视而危害患者。

为此,免疫疗法应运而生,免疫疗法的主要原理是通过抗体结合免疫抑制分子(如大家熟知的CTLA-4和PD-(L)1)。抗体与这些免疫抑制分子结合后可使免疫细胞重新激活,进而进攻癌细胞。

问题在于,虽然PD-1等免疫检查点抑制剂对一些患者的肿瘤治疗显示了优秀的疗效,但仍然对很多患者无法显示疗效,失效率太高一直是让研究人员头疼的一件事。进一步的研究证实,肿瘤细胞产生的转化生长因子(TGF-β)是导致免疫检查点抑制剂失效的一个重要原因。

由此,药物开发人员尝试在一代PD-1抑制剂的基础上引入TGF-β,由此可以同时抑制PD-1和TGF-β,二代PD-1(Y-traps)也由此应运而生。

2、新型双功能免疫治疗药物Y-traps的诞生

正如上面所说,二代PD-1--Y-traps这一概念的提出,正是基于研究人员在寻找PD-1等免疫检查点抑制剂失效原因的时候发现TGF-β在期间行驶的重要作用。

所谓Y-traps(图2),Y代表的是Y形,该类药物是将靶向抗体(如PD-1)融合到"traps"中,并同时基于TGF-β受体也设计了一个"traps", 由此屏蔽TGF-β的作用。

Y-traps这一概念被提出之后,迅速得到了广大药物开发人员的认可,相继有CTLA-4抗体与TGFβ trap融合药物、PD-L1抗体与TGFβ trap融合药物被报道,并在临床上得到了疗效确认。

3、关于M7824

M7824是德国默克公司MERK研发的一种双功能融合蛋白,由程序性死亡配体1(PD-L1)单克隆抗体融合人转化生长因子-β(TGF-β)受体II的细胞外结构域组成。因此,该药物可以作用TGFβ和PD-L1两个靶点,大地提升药物的有效率,并降低联合用药的副作用。该药物的临床前动物试验数据显示有效率高达100%。

在今年(2018年)召开的ASCO年会,默克公司公布的关于M7824二线治疗非小细胞肺癌的I期临床研究结果显示,对于PD-L1≥80%的强阳性患者来说,M7824的客观缓解率高达71.4%,具有巨大的治疗应用潜力。

小结

现有的研究结果显示了二代PD-1药物--M7824在有效率、副作用上较原有检查点抑制剂均有显著优势。我国肿瘤患者基数巨大,目前,PD-1药物Opdivo(O药)和Keytruda(K药)已经在我国上市,迅速惠及到了我国很多肿瘤患者。而M7824作为PD-1加强版,其市场潜力巨大,可以预知,该药物一旦在我国上市,定可迅速夺取我国肿瘤药物市场份额。
