盘点:生物标志物——让医疗更精准

生物标志物作为最直接快速有效的诊断手段,其筛选与获得可在疾病诊断、发展、治疗、以及疗效监测等多个方面发挥重要的作用。同时也是药物开发的重要靶标。目前已有多种技术平台被应用于生物标志物研究以及多种前沿技术在内的手段与方法,都为快速获得及筛选生物标志物带来了极大的可能。随着高通量基因组学、蛋白质组学等的不断进展,生物标志物包括的种类也越来越多,例如SNP、外泌体、miRNA、lncRNA等都被列入生物标志物的行列。这里小M整理了2016年以来关于生物标记物的重要研究进展与大家一同分享。



【1】 4个生物标志物或可提高卒中风险的预测能力


根据一项新的分析,4个生物标志物水平升高——C-反应蛋白(CRP),肿瘤坏死因子受体2(TNFR2),同型半胱氨酸和血管内皮生长因子(VEGF)——可增加事件性缺血性卒中风险(ISS)。

这项分析包括3224名参与者,参加第7轮的检测,包括15个炎症和内皮功能障碍相关生物标志物的评估。在一个调整了年龄和性别的模型中,他们发现CRP,TNFR2,总同型半胱氨酸和VEGF的每个SD增加均与缺血性卒中风险相关。

在同时调整了弗雷明汉卒中风险预测其他变量——收缩压、高血压治疗、吸烟、糖尿病、心血管疾病和房颤——的模型中,发现除CRP外的其他生物标志物仍很显着。

将这4个生物标志物纳入到弗雷明汉卒中风险预测中可提高其预测缺血性卒中风险的能力,连续净重新分类指数(NRI)为0.34。(文章详见—Neurology:4个生物标志物或可提高卒中风险的预测能力



【2】生物标志物突破可能改进帕金森病的治疗

来自佛罗里达大学的研究人员使用功能性磁共振成像揭示帕金森病和相关疾病导致大脑活动进行性衰退的区域。这种追踪疾病进展的新型无创方法,可能有助于评估实验性疗法,从而延缓或停止疾病的发展。

研究人员使用功能性磁共振成像评估了大脑中5个涉及运动和平衡的关键区域。基线研究一年后,研究中46位帕金森患者表现出两个区域的功能下降:初级运动皮层和硬膜。该研究中的帕金森相关障碍评估也显示出下降:13位存在多系统萎缩的患者表现出3个区域的活动下降,19位存在进行性核上性麻痹的患者表现出全部5个区域的下降。34位健康对照者的大脑活动没有改变。(文章详见——Neurology:生物标志物突破可能改进帕金森病的治疗



【3】鉴别出750多个生物标志物 助力癌症早期筛查

近日,刊登在国际杂志EBioMedicine上的一项研究报告中,来自英国谢菲尔德大学的研究人员通过研究在血液中鉴别出了788个生物标志物(Biomarkers),这些生物标志物或可用于开发针对一般人群的早期癌症筛查测试技术。

研究人员表示,他们以过去5年里发表的1.9万篇研究论文开始他们的研究,这些研究论文都基于生物标志物对血液进行了相应的调查,而且系统性的评价方法(排除了少于50个病人的研究)将上述研究论文最后限定到4000个,这样一来研究者就可以对最终的生物标志物列表进行汇编了。
研究者调查了是否一种划算的筛查测试技术可以被用于一般人群来帮助鉴别早期的癌症患者;下一步他们将对生物标志物的详细信息进行分析,来检查这些生物标志物是否能够帮助进行准确的筛查检测,在早期阶段生物标志物将会根据癌症类型被分组,被验证的生物标志物也将用于临床研究中,同时研究者也将利用癌症患者的样本,并以健康个体为对照来检查其鉴别出癌症存在的有效性。

最终被证实可以成功用于筛查的生物标志物将会被考虑进入临床试验中,来观察是否这些筛查检测技术可以更好地在实际工作中使用。研究者认为,一旦验证过程和临床研究完成的话,我们将会得到一组包括50个生物标志物的系列,而且最终会进入到临床试验中去,为了完成这项验证过程,这项试验将需要6-8年时间来完成,但从理论上来讲,研究者将会在三年内对高风险的个体开展试验。(文章详见——EBioMedicine:鉴别出750多个生物标志物 助力癌症早期筛查

【4】生物标志物可鉴别克罗恩病和溃疡性结肠炎

根据加拿大的研究人员的报告,新发现的生物标志物可能有助于炎症性肠病(IBD)的诊断和指导治疗。

研究人员对来自99名患者的活检进行了蛋白质组学评估,这些患者未进行过任何治疗。对3500多种蛋白质进行量化后,他们确定了一组(5种)区分IBD患者与对照组患者的蛋白质,精确度为95.9%;和一组(12种)区分CD与UC的蛋白质,精确度为80%。

此外,研究人员还确定了116种与疾病的严重程度相关的蛋白质,其中4种(包括内脂素和金属硫蛋白-2)是上述两组蛋白质的成员。(文章详见——Gut:生物标志物可鉴别克罗恩病和溃疡性结肠炎



【5】诊断阿滋海默病的脑脊液和血液生物标志物

一想系统性回顾分析表明,新出现的脑脊液生物标志物  NSE, VLP-1, HFABP, 和YKL-40与阿兹海默病有中度相关,血清Aβ42 和Aβ40 与AD无关。由于它们的稳定性,脑脊液中 T-tau, P-tau, Aβ42, 和NFL 应该推荐用于阿滋海默病的临床实践和临床研究中。

研究人员回顾了1984年1月1日到2014年6月30日PubMed和科学网络上的关于反应神经退行性变(T-tau,NFL,NSE,VLP-1,andHFABP),APP代谢(Aβ42,Aβ40,Aβ38,sAPPα,andsAPPβ),神经纤维缠结病理(P-tau),血脑屏障功能(蛋白比值),星形胶质细胞活化(YKL-40,MCP-1,GFAP)的脑脊液和血液生物标志物。

在PubMed中识别了4521条记录,科学网站识别了624条记录,分析中共231篇文章包含了15699个AD患者和13018个对照组人员。表现良好的主要的生物标志物以区别阿兹海默病与对照组的是CSFT-tau,P-tau,和Aβ42。由AD导致的轻度认知功能障碍与稳定性认知功能障碍患者中的差异也很大,。除此之外,CSF NFL 和血清 T-tau在区别AD与对照组有大效应, CSF NSE, VLP-1, HFABP,和 YKL-40有中度效应。其他评估的生物标志物只有很小的效应或在对照组与患者样本中根本没有区别。(文章详见——Lancet neurol:诊断阿滋海默病的脑脊液和血液生物标志物,meta分析

【6】新型生物标志物ABC—房颤抗凝出血风险评分系统效果较好

发表于Lancet的一项研究在一个房颤病人抗凝治疗的大型队列中,从内部和外部验证了ABC-出血评分系统,使用年龄,出血史和三个生物标志物(血红蛋白,cTn-hs, 和GDF-15或抑光素C/CKD-EPI)。ABC-出血评分比HAS-BLED和ORBIT 显示出了较好的预测效果,应该作为房颤病人抗凝治疗的决策支持。

研究人员建立并从内部验证了一种新的基于生物标志物的评分,预测大出血风险,在ARISTOTLE试验中,14537个房颤病人随机分配到阿哌沙班VS华法林组;在RE-LY试验中我们从外部验证了,8468个房颤病人随机分配到达比加群VS华法林组。

最重要的大出血预测因子是生物标志物分化因子-15(GDF-15),高敏心肌肌钙蛋白T(cTnT-hs) 浓度增加,及血红蛋白,年龄,和前次出血。ABC出血评分(年龄,生物标志物[GDF-15, cTnT-hs, 血红蛋白],临床病史[前次出血])比传统的HAS-BLED和新近的ORBIT评分系统在两个对大出血的推到队列中都有较高的c指数,分别为(0.68 vs 0.61  vs 0.65;ABC-出血vs HAS-BLED, ABC-出血 vs ORBIT)。ABC-出血评分同时在外部验证中产生了较高的C指数(分别为0.71  vs 0.62 vs 0.68,ABC出血 vs HAS-BLED,ABC-出血 vs ORBIT)。调整的ABC出血评分选择性的使用生物标志物(血红蛋白,cTnI-hs,抑光素C,或肌酐清除率)也优于HAS-BLED 和 ORBIT评分。(文章详见——Lancet:新型生物标志物ABC—房颤抗凝出血风险评分系统效果较好



【7】外泌体或可作为黑色素瘤诊断和预后的生物标志物

发表在Clinica Chimica Acta的研究,西班牙科学家确定并评估了胞外体中黑色素瘤标志物的存在,包括MIA、S100B和酪氨酸酶相关蛋白2(Tyrp2)等,及胞外体中的这些标记物的潜在临床应用价值。

实验收集了黑色素瘤IV期患者、非黑色素瘤患者和健康对照人群的血清样品。沉淀获得胞外体;对血清、胞外体和无胞外体血清中的TYRP2、MIA和S100B的浓度进行定量。

结果显示,在胞外体中检测到了MIA和S100B,并与血清浓度呈显着相关。黑色素瘤患者胞外体中MIA和S100B浓度显着高于健康对照人群和非黑色素瘤患者。然而,三组人群血清胞外体中TYRP2的浓度没有差异。MIA的ROC曲线分析所得峰下面积(AUC)为0.833,S100B为0.840。胞外体中MIA浓度高于2.5μg/L的患者的平均生存期比胞外体中MIA水平低的患者短。

【8】Diabetes Care:血糖峰值生物标志物1,5-AG与亚临床心血管疾病

1,5-脱水葡萄糖醇(1,5-AG)是葡萄糖峰值的一种生物标志物,与临床心血管疾病相关。然而1,5-AG与亚临床心血管疾病之间的关联是未知的。我们研究了1,5-AG与亚临床心肌损伤(高敏肌钙蛋白T [ hs-cTnT ]评估)和亚临床动脉粥样硬化(由颈动脉内膜中层厚度[CIMT]和颈动脉斑块评估)之间的关系。结果发现在糖尿病患者中,1,5-AG与亚临床心血管疾病有关,尤其是慢性亚临床心肌损伤。

研究人员对10072非糖尿病患者和参加ARIC研究的681的糖尿病患者测定了1,5-AG、hs-cTnT、CIMT和颈动脉斑块等参数。结果,在糖尿病患者中,横截面上低1,5-AG(<6 μg/mL)与高1,5-AG (≥10 μg/mL)相比,与升高的hs-cTnT有关。非糖尿病患者中1,5-AG与厚CIMT或颈动脉斑块的而存在之间的联系不太可靠。对于糖尿病患者,低1,5-AG前瞻性的与6年hs-cTnT 升高有关。校正HbA1c后,所有的联系会强烈的衰减。(文章详见——Diabetes Care:血糖峰值生物标志物1,5-AG与亚临床心血管疾病
