BMJ:肥胖与肾小球滤过率下降风险的关系

近日,顶级杂志BMJ上发表了一篇研究文章,研究人员旨在评估肥胖指标(体重指数、腰围和腰围与身高比)与肾小球滤过率(GFR)下降和全因死亡率之间的关系。

该研究为针对个人参与者数据进行的Meta分析,来自40个国家1970年至2017年间收集的数据组成了研究队列。参与者包含39个普通人群队列(n=5459014),其中21个(n=594 496)具有腰围数据的队列; 6个血管高风险的队列(n=84417)和18个患有慢性肾病的队列(n=91607)。该研究的主要结局指标为GFR下降(估计的GFR下降≥40%,开始肾脏替代治疗或估计的GFR<10mL/min/1.73m2)和全因死亡率。

在8年的平均随访中,一般人群中246607(5.6%)人的GFR下降(18118(0.4%)发生终末期肾病事件)和782329(14.7%)人死亡。调整年龄、性别、种族和当前吸烟后,体重指数30、35和40与体重指数为25的个体相比GFR下降的风险比分别为1.18(95%置信区间为1.09-1.27)、1.69(1.51-1.89) )和2.02(1.80-2.27)。估计GFR的所有亚组的结果相似。在调整其他合并症后,相关性有所减弱,各自的风险比分别为1.03(0.95-1.11)、1.28(1.14-1.44)和1.46(1.28-1.67)。体重指数与死亡之间的关系为J形,体重指数为25的个体风险指数最低。在血管风险高和慢性肾病(平均随访6年和4年)的队列中,较高体重指数与GFR下降之间的风险关联弱于一般人群,体重指数与死亡之间的关系也呈J形,体重指数为25~30之间的个体风险最低。在所有队列类型中,高腰围和腰围与身高的比值与GFR下降之间的关系与体重指数相似;然而,与体重指数相比,增加的死亡风险与腰围或腰围与身高比无关。

由此可见,体重指数、腰围和腰围与身高比增加是估计的GFR水平正常或降低的个体出现GFR下降和死亡的独立危险因素。
