ASCO 2013：HPV阳性口咽癌患者的配偶口腔HPV感染不会增加（HOTSPOT研究）

    很多人乳头瘤病毒(human papilloma virus, HPV)阳性口咽癌(oropharyngeal cancer,
OPC)患者的配偶会担心他们患HPV相关肿瘤的风险。配偶的口腔人乳头瘤病毒传播史(Human Oral Papillomavirus Transmission in Partners over Time, HOTSPOT)研究是第一次大规模检测HPV诱发的口咽癌患者及其配偶口腔中HPV感染情况，该研究发现，与普通人群相比，配偶口腔HPV感染的阳性检出率不会增加。
    HPV感染在美国男性和女性中均很常见，但是绝多数个体感染HPV后不会患上肿瘤。然而在过去20年里，HPV阳性的头颈癌发病率显著增加，尤其是在美国非西班牙语的白人男性中。
    领导该研究的发起人、马里兰州巴尔的摩约翰霍普金斯大学流行病学副教授、PhD、MPH、MS-Gypsyamber D‘Souza指出，HPV阳性的口咽癌患者及其配偶经常担心HPV经口腔转播，并想知道配偶患肿瘤的风险。这项研究结果证实口咽癌伴侣的口腔HPV感染率没有增加，他们患HPV相关口咽癌的风险仍然很低。相处多年的夫妻可能已经共享对方的感染，不需要改变他们身体亲密方式。
    这项研究入组了147例HPV-OPC患者和83个配偶/伴侣。OPC患者主要是男性，伴侣主要是女性。该研究中OPC患者的中位年龄是56岁。检测口腔中HPV DNA的样本是采集疾病确诊时和1年后的30秒钟口腔清洗剂和漱口水。检测口腔清洗剂样本中包括HPV16在内的36个不同HPV亚型，HPV16是多数
HPV-OPC和其他多种肿瘤的罪魁祸首。
    HPV-OPC患者的口腔清洗剂样本中有66%可检出HPV DNA，经过一年的肿瘤治疗后，仅7%的患者仍有口腔HPV感染。患者伴侣的口腔中HPV总体感染率为7.2%。75例女性伴侣口腔HPV感染率为5%，这与普通人群中妇女口腔的HPV感染率相差无几（数值为4%，数据来源于既往报道）。少数男性伴侣口腔中的HPV感染率尽管高于女性人群，但与普通人群中男性口腔感染率类似。作为多数HPV-OPC罪魁祸首——HPV16在HPV-OPC患者的样本中检出率是54%，在女性伴侣口腔中检出率是2.7%，而在男性伴侣未曾检出。总而言之，研究者声称：该发现对女性和男性伴侣提供安心保证：他们患HPV-OPC风险很低。在64%接受可视化口腔检查的伴侣中，未发现癌前病变或肿瘤。
    ASCO发言人和头颈部肿瘤专家Gregory Masters博士指出，HPV是每年发生的数千病例口咽癌、宫颈癌和其他部位肿瘤的罪魁祸首。这项研究提高了我们对HPV阳性口咽癌患者的伴侣感染HPV风险的认识。他确信这一消息对患者及其配偶和伴侣而言，提供了期待已久的安心保证。但是研究者强调，为了更好地理解HPV口腔传播，需要调查更多的年轻夫妇。
