Eur Respir J：呼吸道感染是急性心肌梗死和卒中的触发因素！

由此可见，显示肺炎链球菌和流感病毒具有明显的心血管触发效应。但需要进一步研究非流感呼吸道病毒的血管效应。

虽然急性呼吸道感染可能引发心血管事件，但特定病原体的不同作用尚不清楚。这对指导疫苗政策很重要。

近日，呼吸疾病领域权威杂志Eur Respir J上发表了一篇研究文章，研究人员利用与苏格兰发病率记录相关的国家感染监测数据，确定了2004年1月1日至2014年12月31日期间首次发生的心肌梗死或卒中患者，并在此期间记录了实验室确诊的呼吸道感染。使用自我控制的病例系列分析，研究人员得到了感染后心肌梗塞（n=1227）或卒中（n=762）的调整年龄和季节的发病率比（IRs），并与基线时间进行比较。

研究人员发现肺炎链球菌和流感病毒感染后一周内心肌梗死发生率明显增加：第1-3天调整后的IR分别为5.98（95％CI为2.47-14.4）和9.80（95％CI为2.37-40.5）。感染后卒中发生率同样较高，并且持续升高至28天：肺炎链球菌和流感病毒感染的第1-3天调整后IR分别为12.3（95％CI为5.48-27.7）和7.82（95％CI为1.07-56.9）。虽然其他呼吸道病毒与两项结局的升高点估计值相关，但只有4-7天的卒中估计值达到统计学显著性。

由此可见，显示肺炎链球菌和流感病毒具有明显的心血管触发效应。但需要进一步研究非流感呼吸道病毒的血管效应。
