Radiology:四维多相稳态增强MR在小儿先天性心脏病中的应用

 尽管MRI在小儿先天性心脏病(CHD)中的益处已得到公认,但无法预测的检查时间以及对专家互动式监督的需求限制了其广泛使用。使用四维(4D)多相稳态增强Ferumoxytol MRI(MUSIC)可以产生独立于疾病复杂性的各向同性、高空间分辨率、心脏相位分辨率的三维(3D)图像。在不间断正压通气过程中获得心脏和呼吸门控MRI数据,图像重建可在几秒钟内完成。

四维MUSIC MRI依赖于Ferumoxytol的稳态分布特性来增强血管内信号强度。Ferumoxytol是一种超小的超顺磁性氧化铁纳米颗粒,已被美国食品和药物管理局批准用于缺铁性贫血的临床治疗。由于其长的血管内半衰期和高的弛豫性,Ferumoxytol具有独特的性质及疗效。使用Ferumoxytol并非没有过敏风险,然而迄今为止,诊断性Ferumoxytol应用的安全性良好。

近日,发表在Radiology杂志的一项研究评估了4D MUSIC MRI在小儿CHD的多中心应用的可行性,为4D MUSIC MRI的进一步广泛应用铺平了道路。

本项前瞻性研究对所有患有CHD的参与者在2014年至2020年之间进行了3.0 T或1.5 T的4D MUSIC MRI扫描。从460项总研究中,选择了来自三个地点(n = 60)的等量MRI研究进行详细分析 。使用五分制量表,根据伪影、图像质量以及心内和血管连接的诊断置信度(n = 780)对4D MUSIC的可行性进行了评分。通过使用信号强度曲线评估呼吸运动抑制。使用Bland-Altman分析评估4D MUSIC和二维(2D)电影成像之间的偏差;将4D MUSIC检查的持续时间与当地的CHD标准进行了比较。

共有206名CHD患者接受了3.0 T的MRI检查,254名参与者接受了1.5 T的MRI检查。在选择进行分析的60项MRI检查中(每部位20例;参与者年龄中位数为14.4个月[四分位间距为2.3-49个月] ;33名女性参与者),56名(93%)在疾病复杂度范围内具有良好或出色的图像质量得分(平均得分±标准差:部位1为4.3±0.6,部位2为4.9±0.3,部位3为4.6±0.7; P <.001)。伪影评分与图像质量呈负相关(r = -0.88, P < .001),与呼吸运动抑制呈负相关(P < .001, r = -0.45)。730/780例(94%)心内和血管连接的诊断置信度很高或确定。4D MUSIC、2D电影心室容积与射血分数相关性高(r = 0.72 ~ 0.85;P < .001)。与当地标准MRI相比,4D MUSIC缩短了图像采集时间(44分钟±20 vs 12分钟±3;P < .001)。

本项初步多中心研究结果表明,四维(4D)稳态增强(MUSIC)心血管MRI在不同机构、场强、年龄谱和疾病复杂性的先天性心脏病(CHD)儿童患者中是可行的。在提高时间效率、简化图像采集、以及减少钆蓄积及保护肾功能的情况下,四维MUSIC MRI始终能够获得可靠的诊断图像和极有价值的质量指标。本研究为4D MUSIC MRI更广泛的多中心应用铺平了道路,为其在冠心病中的进一步应用提供了有价值的参考依据。
