循环中极长链饱和脂肪酸(VLSFA)是饮食和新陈代谢的综合生物标志物,可提示新的风险途径和预防心力衰竭(HF)的潜在靶标。人类中VLSFA与HF之间的关联尚不清楚。

近日,血管疾病领域权威杂志JAHA上发表了一篇研究文章,研究人员使用队列研究设计,在血管健康研究中分析了连续测量的血浆磷脂VLSFA与HF事件之间的关联。研究人员使用Cox回归调查了3种主要的循环VLSFAs,木质素酸(24:0)、山酸(22:0)和花生酸(20:0)的时变水平与HF事件风险之间的关系。

在4249名参与者的45030人次每年随访中,研究人员确定了1304例HF事件,其中489例射血分数正常,310例射血分数减少。调整主要HF危险因子和其他循环脂肪酸,每个VLSFAs的较高水平与较低的HF发生风险相关(趋势P≤0.0007)。比较最高五分位数与最低五分位数的风险比对于24:0为0.67(95%置信区间为0.55-0.81),对于22:0为0.72(95%置信区间为0.60-0.87),对于20:0为0.72(95%置信区间为0.59-0.88)。在由性别、年龄、体重指数、冠心病和糖尿病定义的亚组中,这种关联相似。在具有射血分数数据的患者中,在射血分数正常和射血分数减少的患者中这些关联看起来相似。

由此可见,较高水平的循环VLSFA与老年人发生HF事件的风险较低有关。这些新的联想应该推进进一步探究VLSFA在HF中的作用,包括相关的新风险途径。
