既往研究显示,使用他汀有助于稳定脆弱冠脉粥样斑块和延缓病变进展,那么病变处血流动力学有无变化?

近日,上海交通大学附属第六医院学者进行的研究发现,对于轻重度冠脉狭窄冠脉病变,服用他汀类药物,有助于潜在改善血流动力学状态,主要体现在非钙化病变。

这项研究应用的检查手段是CT 冠状动脉血流储备分数(CT-FFR),研究者同时认为,监测他汀类药物对不同类型斑块的治疗作用,CT-FFR或是好手段。

研究显示,他汀治疗1年多,对于非钙化斑块、钙化斑块和混合斑块三个亚组,非钙化斑块组CT-FFR较基线显著减小,而其他两组未见明显变化。

而三组在其他参数,比如最小管腔直径、直径狭窄、总病变长度、钙化斑块体积、非钙化斑块体积等均未见明显变化。

在校正了传统心血管危险因素和冠心病临床特点后,应用他汀还是与降低非钙化斑块患者CT-FFR相关。

这一结果也与之前研究相一致:他汀治疗对非钙化斑块更有效。

研究共纳入152例既往无心梗、血运重建病史患者,平均年龄67.1岁,其中100例为男性,平均随访13.9个月。
