该MR分析发现了由基因决定的降低血压与较低的AD风险有关的证据,CCB被确定为预防AD的一个有希望的策略。

阿尔茨海默病(AD)的发病率正在上升,进一步增加了社会和经济负担。在没有任何治疗干预的情况下,针对可改变的风险因素的预防策略是很有前途的方法。高血压已经成为AD的一个潜在风险因素。抗高血压药物(AHMs)也被强调为预防AD的优先再利用候选药物。然而,观察性研究的推论受到了残余混杂、反向因果关系和检测偏差的限制。实施大规模随机临床试验(RCTs)的困难也限制了对这种关联的探索。


在观察性研究中,利用遗传变异估计危险因素的因果效应的一种新方法是孟德尔随机化(MR)。由于受孕时基因的随机分配,MR克服了观察性研究的核心缺陷,评估了风险因素的终身暴露,因此,它可以澄清潜在的因果关系。

最近,有两项研究利用MR分析估计了BP水平对AD风险的因果效应。一项研究采用了24个单核苷酸多态性(SNPs),表明遗传预测的较高收缩压(SBP)与较低的AD风险有因果关系。然而,另一项研究利用了更多的SNPs(N = 93),发现SBP水平和AD之间没有明显的关联。

Andrews等人也发现SBP增加的多基因风险评分(PRS,优先考虑推定的因果风险因素评分)与AD风险之间没有关联。研究结果之间的差异可能在于所包括的SNPs数量、统计能力和分析偏差的不同;

因此,迫切需要有更大样本量的高质量研究来证实这个问题。此外,药物靶点的表达和功能可以受到编码这些靶点的基因内部或附近的变异的影响。

因此,药物作用的效果可以通过其蛋白靶点基因的遗传效应来预测,正如以前应用于降血脂药物一样。然而,以前只有一项研究采用MR方法来研究AHMs对AD风险的影响,表明通过AHMs的蛋白靶点来降低SBP不太可能影响AD的发病风险。

在此,考虑到以前的MR分析产生了相反的结果,而且现在有更大的血压特征和AD的数据库,藉此,复旦大学的Jin-Tai Yu团队,进行双样本的MR分析,以全面评估遗传决定的血压和抗高血压药物类别的遗传代理对AD风险的因果效应。

他们从BP性状的全基因组关联研究和编码AHM靶点的基因中的降压变体中提取遗传代用品。用反方差加权法作为主要模型计算估计值。进行MR Egger回归和撇除分析(MR Egger regression and leave-one-out analysis)。

结果。有有限的证据表明,基于400/398个单核苷酸多态性(SNPs)的遗传预测SBP/舒张压水平分别影响AD风险(所有P>0.05)。β受体阻滞剂(1个SNP)、血管紧张素受体阻滞剂(1个SNP)、钙通道阻滞剂(CCB,45个SNP)和噻嗪类利尿剂(5个SNP)的合适遗传变体被确定。

CCB的遗传代理[几率(OR)=0.959,95%置信区间(CI)=0.941-0.977,P=3.92×10-6]和AHM的整体使用(OR=0.961,95%CI=0.944-0.978,P=5.74×10-6,SNPs=52)与AD的低风险有关。

没有发现明显的异质性和方向性多态性(所有P>0.05)。其他分析部分支持这些结果。没有一个SNP在驱动观察到的效应。

该MR分析发现了由基因决定的降低血压与较低的AD风险有关的证据,CCB被确定为预防AD的一个有希望的策略。
