Alzheimer & Dementia:“游神”、“难以专注”、“多动”?小心老年痴呆!

ADHD与AD和任何痴呆症都有跨代关系。

注意力缺陷/多动症(ADHD)是一种常见的神经发育障碍,其特点是持续注意力差、冲动控制能力受损和多动。一项荟萃分析研究估计,全世界60岁以上人群的老年痴呆症(AD)的年龄标准化患病率为5%至7%。

很少有研究对ADHD和AD之间的关系进行探讨,而且仅有的研究之间也存在相互矛盾的结果。一项生态学研究利用美国州级住院出院数据发现,前驱ADHD会增加15%AD风险。另一方面,一项对阿根廷布宜诺斯艾利斯一个医疗保险团体的个人进行的病例对照研究表明,前驱ADHD与AD无关。

到目前为止,还没有大规模的纵向研究来探讨这种潜在的联系,因为这样的研究需要从诊断ADHD到诊断老年AD的很长一段时间的跟踪。首先,ADHD可能通过不良健康结果增加晚年痴呆的风险。例如,一项荟萃分析的回顾确定了与痴呆有关的七个可改变的风险因素,包括糖尿病、中年高血压、缺乏运动、抑郁症、吸烟和低教育程度都是ADHD的后果。其次,成人ADHD可能会模仿AD的认知症状(包括记忆力下降和注意力不集中)。

到目前为止,最大的全基因组关联研究(GWAS)未能检测到ADHD和AD之间的任何遗传重叠。为了全面评估ADHD这一神经发育障碍与AD或任何痴呆、神经退行性疾病之间的关系,来自瑞典卡罗林斯卡学院医学流行病学专家开展了相关研究,结果发表在Alzheimer & Dementia杂志上。

研究人员使用瑞典国家登记册将1980年至2001年出生的全国性队列(指数人)与他们的生物亲属(父母、祖父母、叔叔/阿姨)联系起来,使用Cox模型来研究跨代的关联。

在2132929名索引者的亲属中,有3042名父母、171732名祖父母和1369名叔叔/婶婶被诊断为AD。与没有ADHD的索引者的父母相比,ADHD患者的父母患AD的风险增加55%(HR=1.55,95%CI: 1.26-1.89)。

这些关联随着遗传亲缘关系的降低而减弱,但祖父母与AD的关联仍增加11%(HR=1.11,1.08-1.13),叔叔/阿姨的关联与祖父母相似,但没有统计学意义(HR=1.15,0.85-1.56)。

试验流程

按亲属性别分层的估计值显示,ADHD患者的母亲(1.77,1.23-2.54)比父亲(1.46,1.16-1.84)有更高的AD风险。按索引者性别分层的分析显示,父母对男性索引者(1.53,1.18-1.99)和女性索引者(1.55,1.16-2.09)的估计值相似。当考虑到发病年龄时,患有ADHD的指数人的亲属患早发AD的风险高于患有ADHD的指数人的亲属患晚发AD的风险。

在父母中,与早发AD的关系(1.69,1.34-2.13)比与晚发AD的关系(1.20,0.82-1.77)强。祖父母的情况也是如此,即与早发AD的关系(1.27,1.19-1.36)强于与晚发AD的关系(1.09,1.07-1.11)。叔叔/阿姨以及与任何痴呆症的关联也有类似的模式。

在三个亲属队列中,ADHD与AD和任何痴呆症之间的关系

在祖父母和叔叔/阿姨中,这种关联有所减弱,但仍然升高。早发的AD的关联比晚发的AD更强。对任何痴呆症都有类似的结果。

综上,ADHD与AD和任何痴呆症都有跨代关系。这些关联随着遗传关系的减少而减弱,表明ADHD和AD之间有共同的家族风险。
